🐣نجمع معلومات عنacute sinusitis …… اول شي حط فبالك acute يعني اقل من 1month 🧠👉🏻 ثاني شي اهم سبب له هو viral infection 🔥 وكذلك إذا كان سببه bactérial فعلى الاغلب هي strep.pneumonia 🧠👉🏻 مرات تنتقل anaeróbic bactéria من الأسنان إلى malliary sinus ( طبعاً حط فبالك انه malliary sinus هي اكبر sinus ولذلك تكون اكثر وحده معرضة للانفكشن 🐥مثل الأخو الجبير كل مشكلة توكع براسه 🙈) المريض راح يجيك بأعراض :-
✨nasal obstrution
✨post.nasal drip
✨rhonorrhea,discharge
✨reduce tast ,smile
✨headache In morning but relief at afternoon
✨pain &swelling & tender 👇🏻👇🏻

طبعاً الاعراض ممكن تكشف التشخيص بس مرات ادز ‼️المريض على X-ray حتى اعرف منو sinus المتضررة وادزه على post.rhinoscopy حتى اشوف post.nasal drip ……

🌟 اول شي اسوي له ABCD نوب اسوي assessment واكوله نزل رأسك (حتى لا يرجع الدم للpharynx ) واضغط على خشمك بأيدك وحط عليه كمادات ثلج …نوب حدد هل هو anterior او posterior (الي كدام pyriform هذا ant والي وراه post ) فراح اسوي جذي :-
إذا ذولي كلهم فشلوا واني كنت محدد نقطة bleeding (direct ) راح اكوي cuterilization لكن إذا ما كنت اعرف bleeding point راح اخدر المريض G.A و اسوي اكزامنيشن حتى أدورها ……… إذا كل هذا ما فاد راح اسوي ligation بواسطة endoscopy (ESPAL) لل :
✨maxillary.A
✨ethmoidal.A
✨internal carotid.A
✨sphenopalatine.A
✨وطبعاً بهيجي حالة المريض خسر مقطورة دم فراح انطي blood transfusion & vit.K,C & aminocaproic
✨مرات يصير الbleeding بسبب septal devation فالعلاج راح يكون septal surgery
