✅مدرات الثايازايد لها تأثير فريد عن غيرها والسبب يعود لموقع عملها لذلك لا تستغرب أن وجدتها تستخدم لتقليل البول كما في مرضى الساد SIADH👏🏻

✅الألداكتون spironolacton هو prodrug ولكنه اكتف ويتحول الى مور اكتف canrenone لذلك اي مرض او خلل في الكبد يعرقل تحويله يقلل فعاليته👏🏻

✅اذا كنت تعرف ان كلما زاد الدواء free زاد التركيز وكلما زاد المرتبط بالبروتين (الألبيومين) قل التركيز.. فاالمدرات تخالفك الرأي وتقول كلما زاد الارتباط بالبروتين زادت فعاليتها وضمنت عملها وتوصيلها سالمة الى الكلى 👏🏻

✅لمعرفة ان المدرات في سكة المقاومة لديك 3 طرق لمعرفة ذلك..

✅اولا فشل جرعة فموية من مدر الفورسامايد بتركيز 160mg مرتين في اليوم او ما يكافئيها في اخراج ع الاقل 90mmol من الصوديوم وذلك بتجمعي البول في لمدة ثلاثة ايام.

✅ثانيا عن طريق معادلة موجود ع اغلب التطبيقات المشهوره

وذلك بعد اخذ المدر لمدة ساعة الى ساعتين ثم نحسب قمية الصوديوم الخارجه في البول
بتكون اقل من 50meq/L


✅ثالثا test dose
اخذ جرعة IV بتركيز 80mg من الفورسامايد وتشوف تركيز الصوديوم بالبول في 8ساعات ان كانت النتيجة اكبر من 50mmol يتعبر Diuretic sensitive patient

وان كانت اقل من 50mmol يعتبر Diuretic resistance patient


ملاحظة.
الثانية والثالثة قد تفضل للمعرفة السريعة واعطى الحل للمشكلة ان وجدت.

✅عشان تحكم ع المدر انه فعال او لا بحيث ترفع الجرعة..

✅في outpatient بعد الجرعة الفموية من 2-4 ساعات بتلاحظ زيادة في اخراج البول..

✅في inpatient خلال 6 ساعات بتزيد كمية البول مالم ارفع الجرعة

✅اما بالنسبة للوزن ففي الmassive edema لا نحدد الوزن ولكن بعد زوال الاحتباس واصبح المريض مستقر يجب الا يزيد نقص الوزن عن 0.5kg /d

لأن هذه الكمية تعادل نقصان
500ml /d
من السوائل وبالتالي حدوث hypovolemia وhypotention

✅مهم تعرف ان وجود احد مدر من الloop او الthiazid بالدم وعدم انتقاله للكلى لن يعمل وكل النتيجة هي اثار جانبيه لماذا؟

لانه مكان عملها في الاساس يبدأ من وصولها الluminal side فمهم انتقالها عن طريق الOAT

✅بعكس مدرات البوتاسيوم التي تباشر عملها من الدم


✅ملاحظه..
معظم تداخلات المدرات تأتي من الOAT
امثلة ع ذلك هي ACEI ARBs statins...

✅كل Thiazid ممكن ترفع.LDL وT. G ماعدا ال indapamide وع المدى القصير خاصة..

antidote for heparin

✅اولا لازم نشوف كمية الهيبارين المعطاه اخر ساعتين الى ثلاث ساعات.. ثم
نعطي نصفها بالإضافة يعني
ثم ربعها!

♻️فمثلا لو مريض بيأخذ 1000unite /h
كم بتكون جرعة protamine هنا ؟

بنقول 1000+500+250=1750unite

والمعروف ان كل وحده من الهيبارين بنغطيها ب100mg من protamine

اذا 1750÷100=17.5mg

وبالتالي الجرعة هنا بتكون 17.5mg

💠ملحوظه
انها الmaximum dose 50mg
بإستثناء CABG

💠أيضا ممكن نعتمد ع الوقت لو كان معروف كما موضح بالصورة التالي👇
