✅statin associated muscle symptom (SAMS)

✅من اشيع مشاكل أدوية الستاتين هي وهن العضلات بالاضافة الى تأثيرها ع الكبد وتصنف لعدة مصطلحات ومراحل تصل الRhabdomyolysis وايقاف الدواء

لذلك لازم نحسب SAMS CI SCOR نحسب الاسكور ونمشي عليه.

✅For patients with a low SAMS-CI score (2 to 4):

•Discontinue the statin.

•Evaluate for other causes of symptoms, including depression and anxiety or medication-related side effects.

•Wait for symptom resolution.

After symptom resolution:

✅IF patient was not on a high-intensity statin, restart the same statin at a lower dose

✅ IF the patient was on a high-intensity statin, switch to an alternative high-intensity statin (atorvastatin, rosuvastatin) and address concerns about side effects.

✅ IF the patient experiences adverse muscle symptoms on the alternative high-intensity statin, we recommend switching to fluvastatin, pravastatin, or pitavastatin.

✅ IF the patient was already on a low daily dose of a statin, consider alternate-day dosing 'Alternate-day dosing

•Assess for drug interactions, including those related to addition of a new medication or a change in administration of a long-term medication that may account for symptoms.

•Assess for comorbidities which may account for or exacerbate symptoms (vitamin D deficiency, hypothyroidism, acute or chronic renal failure, or obstructive liver disease). Correct hypothyroidism or low vitamin D level if present.

After symptom resolution:

✅IF patient was not on a high-intensity statin, restart the same statin at same or lower dose. If patient was already on the lowest dose, consider alternate-day dosing, although this may result in lower efficacy. Alternate-day dosing

✅IF patient was on a high-intensity statin, switch to an alternative high-intensity statin (atorvastatin, rosuvastatin) and address concerns about side effects. ✅IF SAMS-CI score is still high, switch to a statin with different pharmacokinetic properties and/or consider non-statin low-density lipoprotein (LDL) cholesterol-lowering therapy.

✅IF switching statin therapy to an agent with a different pharmacokinetic profile is unsuccessful initiate alternate-day (or less frequent) dosing with careful monitoring. Alternate-day dosing.

✅IF SAMS-CI score is low, consider and evaluate for non-statin-associated conditions or therapies that may be causing symptoms.

✅IF symptoms persist, discontinue all statin therapy and consider non-statin cholesterol-lowering

بينما الnormal saline افضل في تعويض السوائل السبب مقابل كل 1000ml بيوصل للدم 250ml او الringer lactate


✅السبب ان طبيعه الnormal saline الفيزيائي ان لا يدخل الخلايا لذلك تلاقي ال1000ml. توزع فيExtrcellular fluid الى ثلاثين 750ml بين الخلايا interstitial وثلث 250ml في الدم intravascular

✅اما الD5W يتحلل السكر بالجسم الى ثاني اكسيد الكربون وماء والماء حر الحركه في الجسم فتلاقيه متوزع في intracellular 60% و %40 في الExtracellular وال extracellular معروف ينقسم الى ثلثين للinterstitial وثلت للدم intravascular

وانت قسم ال1000ml بتلاحظ ان الواصل للدم منه فقط100ml

🛑خبر حلو لمرضى ضغط الدم المرتفع!

اليوم معانا دواء لساته Investigational ولكنه مبشر جدااا بفضل الله ، اجتاز Phase I Trials بامتياز وحاليا خاضع لاختبارات Phase II

✅الدواء اسمه Zilebesiran

نتائج اول مرحلة من الاختبارات ان الدواء طلع فعال جدا فى انقاص الضغط العالى ، لما اتجرب بجرعة 200 مجم أنقص ال SBP بمقدار 10 مم زئبق وأنقص DBP بمقدار 5 مم زئبق ولما اتجرب بجرعة قصوى 800 مجم حقق ضعف الارقام بمعنى ان SBP نزل 20 مم زئبق بينما DBP نزل 10 مم زئبق .

طبعا لا ننسى ان غرض Phase I اصلا هو Safety وتحديد الجرعات بناءا على الامان والقدرة على تحمل الاعراض الجانبية وكذا يبقى الجرعات تتراوح من 200 الى 800 مجم .

✅بس كذا ؟؟

الحقيقة مش بس الفعالية فى انزال الضغط هى الرائعة، كمان وجدوا ان الفعالية بتفضل مستمرة 6 شهور كاملة !! متخيل يا مان ؟؟ 6 شهور الضغط متضبط وفلة

✅بس كذا؟؟

الحقيقة لا ، التأثير الروعة بيحصل عليه المريض من جرعة واحدة فقط ، حقنة واحدة تحت الجلد بتنزل الضغط بكل فعالية ولمدة 6 شهور كاملة !!! بصراحة مبشر جدااااا واضف الى ذلك ان مفيش آثار جانبية تقريبا اللهم الا احمرار فى مكان الحقن ومش عند كل الناس كمان .

✅حاليا هو خاضع لاختبارات Phase II وبيتم دراسته هنا كدواء اضافى Concomitant يتضاف زيادة على ادوية الضغط الكثير اللى المريض بياخدها ومع ذلك ضغطه مش بيضبط فخلونا نشوف هل النتائج هاتبقى مبشرة زى المرحلة الاولى ولا ايه اللى حيحصل ؟؟

✅الدواء عبارة عن si RNA يعنى Small Interfering RNA وهذا بيكون عبارة عن double Stranded RNA بس قصير ولما المريض بياخده بالحقن بيدخل داخل الخلايا بعد عبوره لغشاء الخلية وبداخل الخلية بيتحد مع بروتين ربانى داخل كل خلايانا اسمه RISC يعنى RNA-Induced Silencing Complex وبعدين بيبدأ ال Double Strand يتحول الى 2 Single Strands . واحد منهم بيسيب ال Complex وذا ميهمناش والاخر بيفضل مرتبط بال RISC وبكذا يبقى حصل تنشيط للكومبليكس Active RISC

الكومبليكس النشيط ده بيروح يرتبط مع mRNA فى السيتوبلازم ويمنع تحويله الى بروتين لانه ببسطة بيكسره وبكذا يبقى منع Gene Expression او بعبارة آخرى خلى الجين صامت وغير قادر على التعبير Silencing وعشان كذا بيسموه Silencing RNA

✅الدواء بيمنع تكوين بروتين فى الكبد اسمه Angiotensinogen وهذا كان بيتحول عن طريق Renin الى Angiotensin I ثم بواسطة ACE بيتحول الى Angiotensin II

الدواء بيمنع الحكاية ذي من المنبع لان اصلا الكبد مابيكون من البداية Angiotensenigen وبكذا ما بيتكون Angiotensin II فى النهاية وذا ال VC رهيييييب وبيرفع الضغط بكل قوته وبأكثر من ميكانيزم .
