اول شي نعرف شنو سبب hypothyroidism ؟🤔
ب thyroid gland تقل كمية T3,T4 يحاول الجسم يزيدهن وبالتالي حيكون مستوى TSH عالي

🤍Treatment
Levothyroxine (T4) is preferred over T3 (liothyronine) or T3/T4 combination products (liotrix)

ترتبط toxicity بشكل مباشر بمستويات T4 وتظهر على شكل
1-nervousness
2-palpitations
3-tachycardia
4-heat intolerance
5-unexplained weight loss
6-Drugs that induce the cytochrome P-450 enzymes, such as phenytoin, rifampin, and phenobarbital, accelerate metabolism of thyroid hormones and may decrease the effectiveness.

📌Hyperthyroidism
-تعني وجود كميات كبيرة من T3,T4 ومستوى TSH يكون قليل لان حيصير negative feedback حتى يقلل من إفراز T3,T4
-يُعد مرض يسمى Graves’ disease أحد أمراض المناعة التي تؤثر على الغدة الدرقية، *هو السبب الأكثر شيوعًا لفرط نشاط الغدة الدرقية

-الهدف من العلاج هو تقليل تخليق أو إطلاق هرمون إضافي وهذا يتحقق من خلال
1-removal of the thyroid
2-inhibition of thyroid hormone synthesis
3-block of hormone release from the follicle

1-removal of the thyroid
-يتم ذلك جراحياً أو تدمير الغدة باستخدام اليود المشع (131D)، والذي يتم امتصاصه بشكل انتقائي بواسطة thyroid follicular cells
-يصاب معظم المرضى ب hypothyroid بعد تناول اليود المشع ويتطلبون العلاج باستخدام levothyroxin

2-inhibition of thyroid hormone synthesis

thioamides, propylthiouracil (PTU) and methimazole
هاي الادوية تثبط both the oxidative processes required for iodination of tyrosyl groups and the condensation (coupling) of iodotyrosines to form T3 and T4

💊دواء PTU يمنع تحويل T4 إلى T3
وهذه الادوية ما لها تاثير على thyroglobulin المخزون ب الغدة
-يوصى به خلال الاشهر الثلاثة الاولى من الحمل *بسبب خطورة استخدام دواء Methimazole حيث يعمل التشوهات الخلقية teratogenic effects
-وهذا الدواء PTU ايضا مرتبط بتأثيرات تتضمن
a-hepatotoxicity
b-rarely agranulocytosis


💊دواء Methimazole يفضل استخدمه على دواء PTU بسبب انه
١-اطول half life مما يسمح باستخدام جرعة واحدة يومياً
٢-اقل adverse effect

3-block of hormone release

جرعة دوائية من iodide تثبط iodination of tyrosine
(Wolff-Chaikoff effect)
ولكن هذا التأثير يستمر لبضعة أيام فقط.

-و iodide يثبط إطلاق هرمونات الغدة الدرقية من thyroglobulin عن طريق آليات لم يتم فهمها بعد.

-يستخدم iodide لعلاج thyroid storm أو قبل الجراحة، لأنه يقلل من الأوعية الدموية في الغدة الدرقية

-وiodide لي ينطي عن طريق الفم مو مفيد بالعلاج طويل الأمد وتتوقف الغدة الدرقية عن الاستجابة للدواء بعد بضعة أسابيع.

🔮Adverse effects

1-sore mouth and throat
2-swelling of the tongue or larynx
3-rashes
4-ulcerations of mucous membranes
5-metallic taste.

Thyroid storm
حالة نادرة تظهر مع اعراض شديدة لل hyperthyroidism
Treatment
نفس ادوية hyperthyroidism إلا ان الادوية تعطى بhigher dose وبشكل متكرر

Beta Blockers such as metoprolol and propranolol
هاي الادوية فعالة في إضعاف التحفيز الودي الذي يحدث في فرط نشاط الغدة الدرقية. لكن لا يمكن اعتبار beta blockers أدوية مضادة لفرط نشاط الغدة الدرقية للأسباب التالية

1- B-Blockers don't change the lab-biochemical test results.

2- B-Blockers don't treat or block all metabolic effects of TH

3- B-Blockers don't change the course of disease
