1- Focus on this patient and answer the following:

A) Diagnosis: sinus

B)Causes: due to rupture of the periosteum wich leads to a soft tissue abscess ➱ channel to the skin ➱ creating a draining sinus.

C) More common in: in children

1)Diagnosis: acute osteomyelitis.

2)Causes: caused by bacteria and rarely by fungi.

3)Common features: there are ➱1)Dead bone tissue (sequestrum), which contain dead lacunae ( without osteocyte)
2) Inflammatory cells
3) ⇩fibrosis

4)Identify each arrow:
*Yellow🟡➱ colonies of gram positive bacteria.
*Green🟢➱ clusters of neutrophils admixed with fibrin.
*Blue🔵➱ trabeculae of dead bone

5)Complications:
1-Toxemia and septicemia.
2- Septic thrombophlebitis.
3- Direct spread to muscles (common) and joint
(rare).
4-chronic osteomylitis.
5- malignancy in sinus

6)Treatment:
Acute osteomyelitis must be treated surgically to drain pus and prevent bone necrosis. Antibiotics suitable to the patient's age and the organism are given to control hematogenous spread and to treat the local infection. In other words, antibiotics save life, and surgery helps save bone.

*Blue🔵➱ active lacunae

4)Complications: 1) pathologic fracture.
2) secondary amyloidosis.
3)endocarditis.
4) sepsis.
5) development of squamous cell carcinoma in the draining sinus tract and sarcoma in the infected bone.

5) Treatment: the combination of antibiotics and surgical drainage.

1) Diagnosis: chronic osteomyelitis

2)Identify the yellow area: gaint cell ( granuloma)

1) Diagnosis: mycobacterial( tuberculous) osteomyelitis.

2)Causes: mycobacterium
1%_3% of individuals with pulmonary or extrapulmonary tuberculosis exhibit osseous infection.

3)Identify the yellow area and the arrows:
*Yellow area🟡➱ caseous necrosis
*Blue arrows🔵➱ gaint cells

4) Treatment: surgically and with antibiotics.

1) Diagnosis: chronic osteomyelitis

*Yellow 🟡➱ gaint cell
*Green 🟢➱ normal lacunae ( life lacunae)

الم تتساءل يوما وانت تذاكر روبن هل قد استطعت استيعاب محتواه أو مداركه

عانيت كثيرا ولم تستطع إكماله

وجدت صعوبات كثيرة في حفظه وتفسيره

هنا الحل في كتاب

يعتبر الملخص الميسر الاروع لمرجع روبن

لكن سيتساءل البعض ما الفرق بين هذا البوكت وبين رفيو روبن
الإجابة كالآتي :

1) يعتبر بوكت روبن ملخص لمرجع روبن و الرفيو ملخص الملخص
2) يحتوي الرفيو على مجموعة متنوعة من الأسئلة بعد كل فصل على عكس البوكت و مرجع روبن
3) العيب الوحيد في هاذين المرجعين هو عدم احتوائهما على صور هستو مثل مرجع روبن لكن شرحهما أجمل وملخص أكثر
4) ينصح به وبشدة من قبل زملائنا الأكبر سنا في كلية الطب من هم في سنة 3 و 4
