🍂 Leukemia is Acumulacion of malignant cell ( blast cell or mature cell ) in the BM or blood

Types of leukemia
- Acute leukemia have blast
- chronic leukemia have mature or immature cell

The general clinical features of leukemia
- Fever
وذلك بسبب انه الخلايا immature تتكسر وتخرج endogenous pyrogens والتي هي مسؤوله عن تنظيم الحرارة

- night sweats & pain and fatigue
وذلك بسبب زيادة metabolic rate للخلايا
- Increased WBCs
In the Acute 👉🏻 20000-100000 /l
In the chronic 👉🏻 100000 - 500000 /l

- Slowly circulation
لذلك الاكسجين ما يوصلش للانسجة بشكل كافي فيحصل ضعف في النظر وطنين في الاذن ومشاكل اخرى

- Vitamin B12 increase


Acute leukemia
- Is present of  over 20 % of blast cells in the bone marrow and over 80 % in the blood
- Or group of hemopoetic malignant characterized by accumulation of malignant blast in the blood or bone marrow causing BM failur

حيث انه يشكل 50% من نسبة كل leukemia


ليش بيحصل BM failur ؟؟
اولاً احنا عارفين انه نخاع العظم يحتوي على barrier مايسمح بالخرج إلا لل mature cell
فلما يحصل خلل في hemopoetic cell في نخاع العظم تنقسم الخلايا بشكل عشوائي وكونت عدد كبير من الخلايا و هذه الخلايا يلي انقسمت تمنع الخلايا mature من الخروج عشان عملت compression حول نخاع العظم ، ففي المراحل الاولى من leukemia لما نجي نعمل للمريض CBC بنحصل عنده leukopenia وسبب كذا يصبح المريض عرضخ لاي عودى وكمان بيكون عنده anemia يلي تكونnormochromic normocytic anemia ويكون عنده ايضاً thrompcytopenia مما يؤدي إلى bleeding
ويسمى هذه المرحلة الاول من السرطان Aleukemic leukemia  

بعد كذا  يزيد عدد الانقسامات للخلايا فال bone marrow barrier بيزيد الضغط عليه وماعاد يتحمل فيحصل BM failur فتخرج الخلايا mature و immature للدورة الدمويه وبعد كذا الى الاعضاء مثل الكبد والطحال فيحصل Splenomegaly و hepatomegaly
وكمان بيحصل infiltration للخلايا الى brain فتسبب meningitis


Calcification of acute leukemia
- acute lymphoblastic leukemia is common in children
- Acute myeloblastic leukemia is common in adults
