هواي جتني اسئلة على prandial , postprandial
ثنيهم نفضللهم الانسولين سريع المفعول rapid-shot او الادوية الفموية الي تكون سريعه المفعول

زين شنو الفرق بينهم ؟

الـ prandial معناها وقت الاكل راح يصعد الانسولين بصوره جبيرة (burst )
والـ postprandial معناها بعد الاكل بس بفترة مو طويلة ، الانسولين هم حيصعد مستوى بس بصوره مو كلش جبيرة

اما الادوية الي يكون مفعولها طويل معليها بالـ prandial (يعني معليها بالاكل) هاي راح تسوي cover للـ fasting state ، يعني بعد الاكل بفترة طويللة

2-علاجات antimetabolite تشتغل على S Phase

3-علاجات antimetabolite تكون ميكانزم مالتهن anti folate agent

4-علاج MTX ميكانيزم مالته تثبيط DHFR

5-علاج pemetrxed عنده اثنين ميكانيزم :

-تثبيط DHFR

Bevacizumab

يعمل عن طريق تثبيط عامل النمو الوعائي الي هو
VEGF (Vascular Endothelial Growth Factor)

شنو هذا العامل؟
عامل مسؤول عن تكوين أوعية دموية جديدة لتغذية الورم
ف من خلال ال bevacizumab رح نثبط هذا الورم ونمنع تكوين اوعية دموية يعني نثبط ال vascularization

طبعا اهم آثاره الجانبية:
HTN
Bleeding
Wound healing problem
مشاكل في التئام الجروح

Pre / post Surgery should be avoided
لازم يتم التوقف عن استخدامه قبل وبعد العمليات الجراحية لأنه يزيد خطر النزيف

تجنب استخدامه مع anthracycline لان ممكن يزيد خطر تسمم القلب cardiac toxicity

Ramucirumab

هذا نفس الفكرة

بس شنو هذا يغلق رسبتر VEGF receptor و بالتالي يمنع ارتباط VEGF ب الرسبتر

نفس الاثار الجانبية
وهمينه ما انطيه قبل وبعد العمليات الجراحية خوفا من النزيف

آلية عمله هو اغلاق رسبتر EGFR الي هو
Epidermal Growth Factor receptor

وشنو يعني واذا غلق هذا الرسبتر؟

لان اكو مادة اسمها Epidermal Growth Factor موجودة بأكثر من نسيج على سبيل المثال بالقولون
هاي المادة ممكن تكون موجودة بكثرة أو تكون نشطة بشكل غير طبيعي
وهذا رح يسبب نمو السرطان وانتشاره بشكل اسرع

فمن نغلق الرسبتر رح نوقف ال growth of tumors وتتفعل عملية القتل المبرمج

آثاره الجانبية (دايس)
Diarhea
Infusion reaction
Electrolyte wasting
Skin rash

ما دام بيه مشاكل جلدية وطفح جلدي
اكيد قبل لا ننطيه لازم ننطي antihistamine حتى نقلل من هاي ال adverse effects
