-Chief Complaint
Passage of blood in stool for 4 days
-History of Present Illness
The child was apparently well until about one week before admission, when he developed watery diarrhea, large in amount, occurring around 5 times per day.
After that, the stool became bloody and was described as resembling currant jelly stool.
-There is no clear history of:
Fever
Cough or runny nose
Vomiting
Abdominal trauma
Similar previous attacks
Family history of bleeding disorder or chronic illness
-General Examination
Ill-looking
Lethargic
Mildly pale
Not cyanosed
Not jaundiced
No edema
-Abdominal Examination
Mild abdominal distension
No visible peristalsis
No marked abdominal tenderness
No palpable abdominal mass
No organomegaly
Positive Dance sign
Percussion: tympanic
Bowel sounds: present / not clearly documented
-Initial Management
NPO
NGT insertion
IV fluids with KCl
Antibiotics
What is the most likely diagnosis in this case? and what are the DD?

Most Likely Diagnosis
• Intussusception (ileocolic type).
Differential Diagnoses
1. Meckel diverticulum
2. Infectious colitis (e.g., Shigellosis, Salmonellosis, Campylobacter infection)
3. Midgut volvulus
4. Henoch–Schönlein purpura
5. Juvenile polyp
6. Anal fissure
7. Necrotizing enterocolitis (less likely because of the patient’s age)
Next Step to Confirm the Diagnosis
• Abdominal ultrasound (US) is the investigation of choice to confirm the diagnosis.
• Typical findings:
• Target (Donut) sign on the transverse view.
• Pseudokidney sign on the longitudinal view.
Management
• If there are no signs of perforation or peritonitis, perform an air contrast enema (or hydrostatic contrast enema):
• Confirms the diagnosis and reduces the intussusception (diagnostic and therapeutic).
• Surgery is indicated if:
• Enema reduction fails.
• There is bowel perforation.
• There are signs of peritonitis or bowel ischemia/necrosis.
• The patient is hemodynamically unstable.

🩺 العلامات والديناميكا الدموية
⚠️ الأعراض
• Hypotension
• Tachycardia
• Cold, clammy extremities
• Pulmonary edema (Dyspnea, crackles)
• Altered mental status
• Oliguria
💉 Management
✅ ABCs + Oxygen ± Mechanical ventilation إذا لزم الأمر
✅ إعادة التروية العاجلة في MI (PCI هو الخيار الأول)
✅ Inotropes (مثل Dobutamine) لتحسين قوة انقباض القلب
✅ Vasopressors (مثل Norepinephrine) إذا كان هناك هبوط شديد في الضغط
✅ Diuretics إذا وُجد احتقان رئوي وكان الضغط يسمح بذلك
✅ Mechanical circulatory support (IABP, Impella, VA-ECMO) عند عدم الاستجابة للعلاج الدوائي

ممنوع استخدام المورفين في الحالات الآتية:
1. Patient with head trauma
• لأن المريض قد يكون لديه زيادة في ICP، والمورفين يزيدها أكثر.
2. Patient with biliary colic pain due to GB stone
• لأن المورفين يسبب زيادة انقباض Sphincter of Oddi.
3. Old patient with hypertrophy of prostate
• لأن المورفين يسبب انقباض Sphincter of the bladder مما يؤدي إلى Urinary retention.
4. Patient with bronchial asthma
• لأن المورفين يزيد من إفراز Histamine مما يسبب Bronchoconstriction.
5. Pregnancy
• لأنه قد يؤخر Delivery.
طيب ايش البديل ؟
• Pethidine
Not cause meiosis
Not cause bradycardia
Not cause U retention
Safe in pregnancy
Not cause histamine release
