Pulmonary Hamartoma –

1️⃣ Definition (Def)

Pulmonary hamartoma is the most common benign tumor of the lung, composed of disorganized but mature tissues normally found in the lung (cartilage, fat, connective tissue), usually presenting as a peripheral, well-circumscribed nodule.

2️⃣ Epidemiology

Most common in adults aged 50–60 years
Slight male predominance
Usually solitary
Often incidentally detected on imaging

3️⃣ Etiology (Aetiology)

Exact cause unknown
Considered a developmental malformation or benign neoplasm
Not associated with smoking or inflammation

4️⃣ Pathogenesis

Arises from mesenchymal cells of the bronchial wall
Slow-growing
Does not invade adjacent tissue
Usually asymptomatic unless large or centrally located

5️⃣ Clinical Presentation (CP)

Often asymptomatic
If symptoms occur (rare):
Chronic cough
Recurrent infections (if central)
Hemoptysis (rare)

6️⃣ Radiographic & Morphology 🔬

Gross Morphology:
Peripheral, well-circumscribed nodule
Usually <3 cm
May contain cartilage, fat, and fibrous tissue
No invasion of surrounding lung parenchyma
Imaging Features:
Coin lesion on chest X-ray
Popcorn-like calcification on CT scan (classic hallmark)
Smooth, well-defined margins
Microscopic Morphology:
Disorganized mixture of cartilage, adipose tissue, connective tissue, and sometimes epithelium
No atypia or malignant features
No mitotic activity

7️⃣ Complications

Rare complications due to size or location:
Airway obstruction (central lesions)
Recurrent infection
Malignant transformation is extremely rare

8️⃣ Diagnosis

Imaging: Chest X-ray → Coin lesion, CT → Popcorn calcification
Histopathology (if biopsy done) confirms tissue types
Usually diagnosed incidentally

9️⃣ Treatment

Observation if small and asymptomatic
Surgical excision if:
Symptomatic
Growing lesion
Cannot exclude malignancy
Prognosis: Excellent; essentially curative with surgery
