📜 Osmotic diarrhea
is due to the presence of poorly
absorbed solutes in the intestinal lumen, such as the
diarrhea accompanying milk ingestion in lactase-
deficient individuals.
🚒 Hypermotility diarrhea
occurs
when fluid passes through the intestines too quickly for
normal absorption.
🎊 Inflammatory diarrhea,
such as that
characterized by ulcerative colitis, is due to damage of
the intestinal epithelial cells. #Secretory_diarrhea
, such as
is occurring in this individual, is a result of more water
entering the intestinal lumen than can be absorbed.

A 38-year-old man comes to the emergency department complaining of an abrupt,
severe epigastric pain.
The patient is a known alcoholic who has been seen in the emergency department
for several fights and accidents related to inebriation. Today he is brought in because
of an attack of pain in his epigastric region after an afternoon drinking binge.
The patient describes his pain as radiating to his back, and he feels nauseated.
The patient recently vomited, and he is sweating profusely ?!

#Acute_pancreatitis🤢
results from the activation of the
pancreatic digestive enzymes while in the pancreatic
duct or the pancreatic tissues. The digestion of the
pancreatic tissues results in an acute severe epigastric
pain. The pain intensity increases when walking or
lying flat and diminishes when sitting upright or lean-
ing forward. Eating can also increase pain. The pain
causes vomiting and strong sympathetic activation,
including sweating and anxiety.

A 26-year-old woman presents with fatigue and loose “sawdust-like” bowel
movements as often as three times per day. She also notes an occasional rash over
her joints that is extremely itchy.
The patient was delivered of a full-term infant 9 months ago. Her symptoms
began approximately 1 month post partum. The patient also has a family history
of autoimmune disease ?

Diagnosis IS
#Celiac_disease

Positive for HLA-DQ2
positive for anti-endomysial antibody

Celiac disease (celiac sprue, gluten sensitivity, wheat
allergy) is a disorder of impaired absorption of most
nutrients.
Glutens are the storage proteins present in
wheat, barley, rye, and oats. It is hypothesized that
glutens stimulate an inappropriate T cell–mediated
autoimmune response in the submucosa of the small intestine

When the villi of the small intestine are damaged,
absorption is impaired, causing more than the usual
1.5 L of chyme to pass into the large intestine. In
addition, the chyme is high in fats and undissolved bile
salts, which stimulate and irritate the colon, causing
diarrhea; it is high in unabsorbed carbohydrates, which
are fermented by colonic bacteria, producing gas and
fatty acids (bloating and steatorrhea).
Approximately 10% of patients with celiac disease
will have dermatitis herpetiformis
