An 80-year-old woman is brought to her primary care physician by her adult daughter
because the woman is losing weight and unable to keep food down.
During the past several months the patient has lost weight and has had difficulty
in swallowing both solid and liquid food. She frequently regurgitates undigested food
both during the day and at night while sleeping. The patient describes a feeling of
“fullness” and discomfort in her chest. She also frequently coughs and aspirates while
eating

VS: T 37°C, P 74/min, R 15/min, BP 130/75 mm Hg
PE: Normal

#LABORATORY_STUDIES 🧪🧪🧪

Chest radiograph: Shows an #air-fluid interface in an
enlarged, fluid-filled esophagus
Barium esophagography: Esophageal dilation, a loss of
esophageal peristalsis, poor esophageal emptying,
and a smooth #symmetrical “bird’s beak” tapering of
the distal esophagus.
Endoscopy: Normal with no sign of distal stricture or
carcinoma
Esophageal manometry: Confirms complete absence
of peristalsis in the lower third of the esophagus and
elevated lower esophageal sphincter pressure with
incomplete relaxation during swallowing

dishes and consumed a moderate quantity of beer. His past medical history includes
an appendectomy at age 18 and a cholecystectomy at age 34.

Diagnosis : GERD 🩺🥇

A 65-year-old woman comes to the clinic complaining of weakness and lethargy.
The patient indicates that for the past 6 years she has had rheumatoid arthritis and
has managed the pain by daily ingestion of a nonsteroidal anti-inflammatory drug
(NSAID). During the past 6 months, she noted a decrease in stamina ?!

A 46-year-old man comes to the clinic complaining of abdominal cramping and severe
diarrhea that has persisted for 36 hours.
The patient has just returned from a camping trip in the mountains. Both of his
companions are also experiencing severe diarrhea. The diarrhea initially had loose
feces but now is composed of mostly watery stools. The patient indicates he has not
eaten in the past 24 hours, but he has consumed 2 L of a sports drink (Gatorade).
The patient does not report nausea or a history of fever ?!
