‏منهكٌ من الأفكار ومن الماضي، أودُ أن أتحرر مني ليلةً لأنام فيها قليلًا.😔
أستيقظ من شرودي، يشربني الليل وأشربُ عتمته.
أشرب خيبتي و دموعي نوبات رجفتي.
أحمل أعباءً يُسقطنّي ثُقلها رّغم جلوسي في المقعد، إلا أنني أسقط سقوطاً مؤلماً.
يؤلم قليلاً أحياناً كوخزة إبرة!
وأحياناً أُخرى يؤلم بشدة كـ فيل يدوسُ على معدتي، أو قلبّي أو روحي، لا أدري حقاً!
ويستغرقني طويلاً بعد ذلك لأجد السكون.😔

*Deep vein thrombosis*

*(DVT)*

🔅 Is a blood clot that develops within a deep vein in the body, usually in the leg.


*DVT* usually occurs in a deep leg vein, a larger vein that runs through the muscles of the calf and the thigh.

* Swelling in one or both legs
* Pain or tenderness in one or both legs
* Warm skin on your leg
* Red or discolored skin on your leg
* Veins you can see
* Tired legs

*What causes DVT?*🤔


Anyone can develop DVT, but it becomes more common *over the age of 40*. As well as age, there are also a number of other
* risk factors*, including:

*1-*Having a *history* of *DVT* or *pulmonary embolism*

*2-*Having a family *history of blood clots*

*3-*Being *inactive for long periods* – such as after an operation or during a long journey

*4-*Blood *vessel damage* – a damaged blood vessel wall can result in the formation of a blood clot

*5-*Having certain conditions or treatments that cause your blood to clot more easily than normal – such as cancer (including *chemotherapy* and *radiotherapy* treatment), *heart and lung* disease,
*thrombophilia and Hughes syndrome*

*6-*Being *pregnant* – your blood also clots more easily during pregnancy

*7-*Being *overweight* or obese

*complications*

👉🏻 such as *pulmonary embolism* is a serious condition that occurs when a piece of blood clot breaks off into the bloodstream and blocks one of the blood vessels in the lungs .

*DVT and pulmonary embolism* together are known as venous thromboembolism *(VTE)*.


The combined contraceptive pill* and *hormone replacement therapy* (HRT)

both contain the female hormone oestrogen, which causes the blood to *clot* more easily.

Intervention for Deep Vein Thrombosis:🔻

nursing intervention for deep-vein thrombosis are in the following:

-Assess for and reports sign and symptoms of Deep Vein Thrombosis.
-Keep the affected leg elevated and comfortable position.

-Administer anticoagulants as ordered to reduce the risk of additional clotting.
Carefully calculate heparin dose.

-Do not mix the heparinized solution with other medications.

-Apply warm compress to affected leg using a 2-hour-on, 2-hour-off schedule around the clock.

-Administer oxygen as ordered in order to maintain tissue perfusion.
-Check and monitor any sign of bleeding.
-Check any sign of complication (Pulmonary embolism).

-Obtain lab orders to monitor APTT, PT and INR.


How to get rid of blood in deep veins ?🔻


1)Wear compression socks (20-40 mmHg) as these socks prevent blood clotting.
2) Walking daily and doing the necessary exercises for the legs and arms to avoid coagulation.
3) Do not sit or lie on one position for long periods.
4) Do not cross the legs while sitting or pressing on the back of the knees.

5)Eat 8 glasses of fluid daily

Psoriasis
 is a long-lasting autoimmune disease which is characterized by patches of abnormal skin. These skin patches are typically red, itchy, and scaly. They may vary in severity from small and localized to complete body coverage. Injury to the skin can trigger psoriatic skin changes at that spot, which is known as Koebner phenomenon.

There are five main types of psoriasis: plaque, guttate, inverse, pustular, and erythrodermic. Plaque psoriasis, also known as psoriasis vulgaris, makes up about 90% of cases. It typically presents with red patches with white scales on top.Areas of the body most commonly affected are the back of the forearms, shins, around the navel, and the scalp. Guttate psoriasis has drop-shaped lesions. Pustular psoriasis presents with small non-infectious pus-filled blisters. Inverse psoriasis forms red patches in skin folds.Erythrodermic psoriasis occurs when the rash becomes very widespread, and can develop from any of the other types.Fingernails and toenails are affected in most people at some point in time. This may include pits in the nails or changes in nail color.

The Median Nerve

The median nerve is a nerve in humans and other animals in the upper limb. It is one of the five main nerves originating from the brachial plexus.

The median nerve originates from the lateral and medial cords of the brachial plexus, and has contributions from ventral roots of C5-C7(lateral cord) and C8 & T1(medial cord).

The median nerve is the only nerve that passes through the carpal tunnel. Carpal tunnel syndrome is the disability that results from the median nerve being pressed in the carpal tunnel.

Structure

After receiving inputs from both the lateral and medial cords of the brachial plexus, the median nerve enters the arm from the axilla at the inferior margin of the teres major muscle. It then passes vertically down and courses with the brachial artery on the medial side of the arm between biceps brachii and brachialis. At first it is lateral to the artery and lies anterior to the elbow joint; it then crosses anteriorly to run medial to the artery in the distal arm and into the cubital fossa.

Inside the cubital fossa the median nerve passes medial to the brachial artery, in front of the point of insertion of the brachialis muscle and deep to the biceps.

The median nerve gives off an articular branch in the upper arm as it passes the elbow joint. A branch to pronator teres may arise from the median nerve immediately proximal to the elbow joint.

Forearm

The median nerve arises from the cubital fossa and passes between the two heads of pronator teres. It then travels between flexor digitorum superficialis and flexor digitorum profundus before emerging between flexor digitorum superficialis and flexor pollicis longus.

The unbranched portion of the median nerve (which arises from the cubital fossa) innervates muscles of superficial and intermediate groups of the anterior(flexor) compartment except flexor carpi ulnaris.

The median nerve does give off two branches as it courses through the forearm:

The anterior interosseous branchcourses with the anterior interosseous artery and innervates all the muscles of the deep group of the anterior compartment of the forearm except the medial (ulnar half,which is supplied by ulnar nerve) half of flexor digitorum profundus and flexor carpi ulnaris muscle. It ends with its innervation of pronator quadratus.The palmar cutaneous branch of the median nerve arises at the distal part of the forearm. It supplies sensory innervation to the lateral aspect of the skin of the palm (but not the digits).

Hand

The median nerve enters the hand through the carpal tunnel, deep to the flexor retinaculum along with the tendons of flexor digitorum superficialis, flexor digitorum profundus, and flexor pollicis longus.

From there it sends off several branches:

1. Recurrent branch to muscles of the thenar compartment (the recurrent branch is also called "the million dollar nerve").[1] Here it provides motor innervation to opponens pollicis, abductor pollicis brevis and superficial part of flexor pollicis brevis.2. Digital cutaneous branches to common palmar digital branch and proper palmar digital branch of the median nerve which supply the:a) lateral (radial) three and a half digits on the palmar sideb) dorsum of the tips of index, middle and thumb3. The median nerve supplies motor innervation to the first and second Lumbricals of the hand.

Down syndrome (DS or DNS), also known as trisomy 21, is a genetic disorder caused by the presence of all, or part of a third copy of chromosome 21.It is typically associated with physical growth delays, characteristic facial features, and mild to moderate intellectual disability. The average IQ of a young adult with Down syndrome is 50, equivalent to the mental age of an 8- or 9-year-old child, but this can vary widely.

The parents of the affected individual are typically genetically normal. The extra chromosome occurs by random chance.There is no known behavior or environmental factor that changes the risk. Down syndrome can be identified during pregnancy by prenatal screening followed by diagnostic testing, or after birth by direct observation and genetic testing.Since the introduction of screening, pregnancies with the diagnosis are often terminated.Regular screening for health problems common in Down syndrome is recommended throughout the person's life.

There is no cure for Down syndrome.Education and proper care have been shown to improve quality of life. Some children with Down syndrome are educated in typical school classes, while others require more specialized education.Some individuals with Down syndrome graduate from high school and a few attend post-secondary education. In adulthood, about 20% in the United States do paid work in some capacity with many requiring a sheltered work environment.Support in financial and legal matters is often needed. Life expectancy is around 50 to 60 years in the developed world with proper health care
