the actual reason why this happens is absurd - every VoIP service by law needs to implement caling 911, and the Microsoft Teams app had a massive fuckup that would create a new PhoneAccount object every time you started the app unauthenticated. This can easily lead to thousands of these duplicate objects created, leading to a bug in the telephony component that prevents you from properly performing the call.
