Felty syndrome (very brief):
A complication of Rheumatoid Arthritis

Classic triad:🔴
Rheumatoid arthritis
Splenomegaly
Neutropenia

Main issue: 👉 ↓ immunity → recurrent infections

Diagnosis: 👉 RA + neutropenia + splenomegaly

Treatment:
Methotrexate
± G-CSF
Rarely splenectomy

Exam tip: ✅
RA + recurrent infections → think Felty syndrome ✔️

This is a classic presentation of Infectious mononucleosis ✅

Key clue:
👉 Positive heterophile antibody test (Monospot test)
This is the hallmark lab finding.

🧠 Why this diagnosis?
Fever + severe sore throat
Fatigue
Lymphadenopathy (especially posterior cervical)
Splenomegaly
Atypical lymphocytes (reactive CD8+ T cells)

Cause:
👉 Epstein-Barr virus

⚠️ Important exam pearl:
Avoid ampicillin/amoxicillin → causes rash
Risk of splenic rupture → no contact sports

أهم الـ clues:
Recurrent abdominal pain (مغص متكرر بدون سبب واضح)
Dark urine after attacks (بول غامق بعد النوبات)
Neuropathy (أعراض عصبية)

💡 التفكير الصح:
لو شفت:
Abdominal pain +
Neurological symptoms +
Dark urine

→ مباشرة فكري في مرض Acute Intermittent Porphyria
✅ الإجابة:

🧠 ليه ده الصح؟
ده مرض في heme synthesis
بيحصل فيه accumulation لمواد سامة زي ALA و PBG

دي بتأثر على:
الأعصاب → Neuropathy
البطن → Severe abdominal pain
البول بيكون غامق بسبب oxidation (ممكن يتحول للون غامق بعد الوقوف)
