Antimalarial drugs are medications used to prevent and treat malaria, a disease caused by Plasmodium parasites transmitted through the bite of infected Anopheles mosquitoes. These drugs work either by killing the parasites in the blood or preventing them from multiplying.

Here are some commonly used antimalarial drugs, categorized by their function and use:

1. For Treatment

Chloroquine – Effective against Plasmodium vivax, P. ovale, and P. malariae. Resistance is common in P. falciparum.

Artemisinin-based combination therapies (ACTs) – First-line treatment for P. falciparum:

Artemether-lumefantrine (Coartem)

Artesunate-amodiaquine

Artesunate-mefloquine


Quinine – Used in severe cases or when ACTs are not available.

Mefloquine – Sometimes used when resistance to other drugs exists.

Primaquine – Used to eliminate liver forms of P. vivax and P. ovale to prevent relapse.


2. For Prevention (Prophylaxis)

Atovaquone/proguanil (Malarone) – Commonly used for travelers.

Doxycycline – An antibiotic also effective for malaria prevention.

Mefloquine – Weekly dose; used in areas without resistance.

Chloroquine – Used only in areas where P. falciparum is still sensitive.


3. Severe Malaria

Intravenous artesunate – The preferred treatment for severe malaria.

Intravenous quinine or quinidine – Alternatives where artesunate is unavailable.


Important Considerations

Drug resistance: Especially with P. falciparum, resistance to chloroquine and other drugs is a major problem.

G6PD deficiency: Primaquine can cause hemolysis in patients with this genetic condition.

Pregnancy: Not all antimalarials are safe during pregnancy—quinine and chloroquine are typically used.

• Severe anemia
• Uncontrolled diabetes mellitus (hyperglycemia, ketoacidosis)
• Uncontrolled hypertension
• Active infection (e.g. chest infection, urinary tract infection, wound sepsis)
• Electrolyte imbalance (e.g. hypokalemia, hyperkalemia, hyponatremia)
• Dehydration / hypovolemia
• Coagulopathy (including patients on anticoagulants not corrected)
• Severe cardiac failure or arrhythmia (not stabilized)
• Severe respiratory disease (e.g. acute asthma attack, COPD
exacerbation)
