Treatment of Choice for Unexplained Infertility
1. First-line:
Ovulation induction (letrozole or clomiphene) + IUI (Best for young couples <35 years).
2. Second-line:
Gonadotropin stimulation + IUI (If oral agents fail, but higher risk of multiple pregnancy).
3. Third-line:
IVF (Most effective, recommended if infertility >3 years or age >35).

Infertility Treatment Summary

1. Azoospermia → The only treatment is donor insemination.
2. Oligospermia → Best treatment is IVF-ICSI.

3. A 24-year-old, primary infertility for 2 years with irregular cycles → Clomiphene citrate induction.

4. A 35-year-old, secondary infertility for 3 years after severe postpartum hemorrhage → HMG/HCG induction.

5. A 26-year-old, secondary infertility and secondary amenorrhea for 5 years with high FSH and LH levels → IVF with ovum donation.

6. A 22-year-old, primary infertility for 5 months, irregular cycles, BMI 40 → Metformin and weight loss.
