Alhamdulillah proud to present my 2nd case report in a pubmed indexed journal!
As someone who had no clue as to how to publish these things, My first report was published last month and Alhamdulillah 2nd one comes just a month later and hopefully another one is in the pipeline!
This Domino effect is due to the constant motivation and push by my mentor Zico sir ❤️
I remember when we started training under him, just at the first month sir gave us topics to start our publications. Though it took a long time for me to put my foot on the pedal and start.
Also thanks to my junior Ibrahim khalil for his constant support!

Gynae Obs এর para and gravida calculation এ তো সবারই ঝামেলা লাগে কম বেশি, so read this.
Para – Total number of pregnancies (not babies, not the present pregnancy) that has crossed the age of viability ( 28 weeks ) irrespective of the fate of the baby.
**Para doesn’t include the present pregnancy. It actually means, the total number of previous pregnancies that has crossed the age of viability irrespective of the fate of the baby.
Gravida – Total number of conceptions (not babies) including the present one, irrespective of the gestational age and site of pregnancy.
পেটে বাচ্চা থাকলে gravida হিসাব করতে হয়, যদি pregnant না হয় তাহলে gravida এর কোন ব্যাপার আসবেনা। Non pregnant মহিলাকে বলে Nongravid। তো ধরা যাক, একটা মহিলার পেটে বাচ্চা আছে এখন। তাহলে এবার সহ আগে উনি মোট কতবার pregnant হয়েছেন সেটা আমরা বুঝতে পারবো gravida থেকে। অর্থাৎ, এটা এই মহিলার যততম pregnancy, তার gravida ও তত। একটা উদাহরণ দেইঃ এক মহিলার আগে দুই বাচ্চা, এখন আবার pregnant। তাহলে এ মহিলার gravida হবে 3rd Gravida।
আর Para হচ্ছে, একটা মহিলার জীবনে মোট কয়টা pregnancy ২৮ সপ্তাহ এর বেশি পর্যন্ত গিয়েছে সেটার হিসাব। বাচ্চা মারা যাক, বা বেচে থাকুক সেটা এখানে দেখবার বিষয় না। কেউ যদি এখন pregnant হয় তাহলে সেটা para তে included হবেনা, শুধুমাত্র previous pregnancy গুলো para তে আসবে।
Para লেখার সময় আমরা সাধারনতঃ আগের মোট কয়টা প্রেগনেন্সি ২৮ সপ্তাহ এর বেশি পর্যন্ত গিয়েছে- এটার সাথে আরো কিছু তথ্য ( যেমনঃ কয়টা IUD, কয়টা neonatal death, কয়টা ectopic pregnancy, কয়টা abortion) যোগ করি।
অর্থাৎ,
Para = (কয়টা প্রেগনেন্সি ২৮ সপ্তাহের বেশি পর্যন্ত গিয়েছে) – (কয়টা IUD অথবা Neonatal Death অথবা পরবর্তীতে মারা গিয়েছে) + (কয়টা abortion, কয়টা ectopic pregnancy)
যাইহোক,কথা বাড়ায়ে লাভ নাই , কয়েকটা Scenario দেখলে ক্লিয়ার হবে ব্যাপারটা।
[ বিশেষ দ্রষ্টব্যঃ কাঁচা নষ্ট হইছে বা বাচ্চা পরে গেছে এর মানে হচ্ছে abortion হয়েছে। ]

কালকে যখন বাসায় আসি ফজর এর আজান দিচ্ছিল।
Neurosurgery তে মানুষ আসে আসলে টাকার ঝলকানি, Elite subject এইসব ভেবে।
পরে দেখে কষ্টের তুলনায় টাকা কিছুই না।
কালকেই এক colorectal surgeon এর গল্প শুনতেসিলাম, Private এর প্রফেসর, দিনে ৮ টা কেইস করে, ২-৩ ঘন্টা লাগে, বাসায় যায় এরপরে।
