একটা ঘটনা শেয়ার করি, সেইদিন একটা রোগির ওটি ( V-P shunt করার কথা ছিল) আটকায় দিসিলাম। He had Glioma grade 4. We call it GBM.
এই টিউমার এর সাথে Death এর নাম লেখা থাকে, Surgery just life prolong করে।

যেই ওটি টা cancel করসিলাম, রোগি তখন semi conscious।
আমার স্যার, Anesthesia এর প্রফেসর সবার কথা ছিল রিস্ক নিয়ে ওটি করতে হবে।

রোগি টার Uncontrolled DM ছিল, আমি ৪ ঘন্টা নিযে aggresively manage করসি RBS কমাইতে। আমার expection wise কমে নায়, রোগির মেয়ে মাত্র ইন্টার্নশিপ শেষ করা ডাক্তার।
সবাই decision নিতে ভয় পাচ্ছিল। মেয়েটাকে আমি বলসিলাম অপারেশান এর রিস্ক এমন যে রোগি নাও উঠতে পারে ওটি থেকে।

পরে ওটি না cancel হয়, আমার D/D ছিল sepsis / HONK
রোগির sepsis ছিল, পরে ধরা পড়লো। ৩ দিন পরে রোগি বাসায় গেসে। ২ সপ্তাহ আগে ডাক্তার  মেয়েটার হলুদ ছিল।
ও আমাকে ছবি দিল, বাবার সাথে।
এইযে সার্জারী করে ৬ মাস হায়াত বাড়ে, আগে ভাবতাম ৬ মাস আসলে কি আর but life এর এই মমেন্ট gula তো priceless. একটা মানুষ তার বিয়েতে তার বাবাকে পাশে পেল। আমাকে অনেক thanks দিসে অইদিন ওটি টা আটকানোর জন্য।

এই intuition কোন god gifted জিনিস না, হাজার হাজার রোগি দেখার পরে আসে।
Internship এ আমি প্রচুর DKA রোগি দেখসি, একদিন night এ RBS 20mmol/L, Insulin drip এ চালায়েও control করতে পারিনাই ওইদিন ও পাইসিলাম ওই রোগি ও sepsis এ ছিল।

তো বেশি বেশি রোগি দেখবা খালি, Every patient is a story. কি হইসে কেন হইসে খুজলেই অনেক কিছু হয়ে যায়।

এই লোকের চাপাতির কোপ এ বুকের pectoralis muscle পর্যন্ত কেটে গেসে।
sternum & rib cage exposed হয়ে গেসিল।

প্রফ এর আগে যে যত practise করবে সে তত fluently করতে পারবে। এই তো।

আমি findings বুঝতে পারার কথা বলসি। This requires hours and hours of learning and seeing patients.

It is to find that murmur that everyone missed, it is differentiating the fine and coarse crepitations!


আমার তো murmur শোনার একটা fascination ছিল।
আমার 3rd year এর CA আমাকে তখন বলসিল তুমি ২০০ টা নরমাল pericardium শুনবা আগে।
তো আমি একদিনেই ২০০ Normal heart sound শুনে ফেলসিলাম 😬
