تحتوي ع mycolic acid بسطحهه فماتتصبغ بسهولة مانكدر نشخصهه هي هي كرام بوزتف او نكتف لان ماتتصبغ بيه بس كيميائيا هي كرام بوزتف لوجود thick peptidogylcane layer

Stain : Ziehl neelsen technique
نستخدم صلغة carbol fusion تصبغ بكتريا احمر وال counter stain هي methylene blue

Another stain : auramine and rhodamine

نستخدم selective media لتنميتهه وتحتوي antibiotic

Media used are :

1..semisynthesis

2..lowenstein jensen

3..broth media الاسرع

تكون slow grow لان ال germination time يكون طويل ( تكاثر بكتريا ياخذ وقت )
3-6 weeks

تنتج delayed hypersensitivity reaction لذلك ال tuberculin test يطول يومين يالله تضهر نتائجة


تنتقل عن طريق ال inhalation droplet

اكو لقاح متوفر الهه هو BCG vaccine

سبب المرض التسببة مو البكتريا نفسهه انما الجهاز المناعي ومقاومة الشخص تضعف

Two principles lesion

1..exudative types

مجرد خلايا المناعية تتجمع حول ال bacilli وتكون edema
2..productive :
تتكون granuloma

طبعا المايكوبكتريا تكون داخل ال macrophage فالمسؤؤل عنهه بالجهاز مناعي هو cell mediated immunity

بعدين تنتقل بالدم لاجزاء مختلفة بالحسم اي عضو بالجسم ممكن تصيبة حتى ال bone
وهي بالدم يسموه miliary tuberculosis

المايكوبكتريا كلنة هي داخل ال macrophage يلي حانت inactive فمن تدخللهه البكتريا تتحول الى active ياهو يحولهه هو interferone gamma
فنلكة تريكزة بالدم عالي احد فحصوات نستعملة للكشف عن مايكوبكتريا

غالبا ال lesion يكون بال base of lung
ويكون البكتريا تكتسبهه وانت صغير فاما جهاز مناعي يقضي عليهه او تبقى خاملة تنتضرك تضعف مناعتمك يالله تهاجمك

العلاج :

1..rifamoin

2..isoniazid

3..pyrizinamide

4..ethambutol

5..streptomycin

يستمر ٩ اشهر

تنتقل اما عن طريق inhalation او direct contact

عندي نوعين منهه

Tuberculoi : positive lepromine test
Low infetive / low bacilli in lesion , cell mediated is dominant
T helper
Lepromatois : negative lepromine test / T cytotoxin

علاج : dapsone و Rifampin و clofazimine

مهم مقارنة بين tuberculod و lepromatous leprosy
