The history of religion, of philosophy, and of the world, teaches us that life is "meaningless" in the sense that it is like a dream to be interpreted. And the legends and the narratives we tell are what explains it. That's how people of the past changed the world: by telling stories, new stories.

That's also why ideas change the world: because ideas are like "empty containers" that mold life in its shape, in the shape of its container.

A patient with chest pain poses one of the most complicated diagnostic challenges. The differential diagnosis is extensive and includes several life-threatening, time-critical, must not miss diagnoses. The differential is best remembered anatomically, giving consideration to structures from the skin to the internal organs.

The physician must avoid the cognitive bias of “playing the odds,” favoring the common benign, rather than the uncommon serious, diagnoses.

The initial pivotal points in the evaluation of chest pain are the duration of symptoms and the patient’s vital signs. Patients with normal vital signs and subacute or chronic chest pain usually seek medical attention in primary care setting while those with acute pain and vital sign abnormalities more commonly go to the emergency department.

Myocardial Infarction (MI) in women

Women, being women, have a different presentation for MI:

- They are more likely than men to present without chest pain. And they use different terms than men, like "burning" and "tender", to describe the pain.

- they often report prodromal symptoms like dyspnea, fatigue, and insomnia which are common presenting symptoms.

- Also, these gender differences become less pronounced with older age.
