Clinical Uses of magnesium sulfate is ABCDE :

(A)cute severe asthma
(B)arium poisoning
(C)onstipation
(D)e pointes ( torsade de pointes ) /Digoxin intoxication
(E)clampsia

Dose of magnesium sulfate :

🔥Torsade de pointes:
Loading dose: 2 gram/15 min if the patient was pulsatile,
and over 2 min if the patient was pulseless

Maintenance dose: 1 g/hr for 24 hr

🔥Acute exacerbation of asthma
In Adult: 2 gram over 20 min, no Maintenance dose.

Toxicity: 💔
Loss of patellar reflux
respiratory arrest so contraindicated in MG

Antidot :🤔
Reversal of magnesium sulfate toxicity
Is ca gluconate 1g 10%

وليش مرات يسوي hypo و مرات hyper ...

هو يحتوي على كمية هائلة من اليود.
كل جرعة 200mg من Amiodarone تحتوي على 75mg من اليود.
هذا اليود العالي يؤثر بشكل مباشر على تصنيع هرمونات الثايرويد.
ليش Hypothyroidism؟

🧠 الفكرة الأساسية: Wolff–Chaikoff Effect
عندما تزداد كمية اليود داخل خلايا الغدة الدرقية→يتم إيقاف تصنيع الهرمونات الدرقية مؤقتاً كـ"استجابة ذاتية دفاعية".
هذا الانقطاع يسمى: Wolff–Chaikoff effect.
يستمر عادة 10-14 يوم فقط.
بعدها، ترجع الغدة للإنتاج الطبيعي بسبب ما يسمى: Escape phenomenon.

📌 في بعض المرضى، لا يحدث الـ Escape!
خاصةً في مرضى autoimmune thyroid disease.
النتيجة؟ استمرار الـ Wolff–Chaikoff effect → Hypothyroidism مزمن.

3. لماذا Hyperthyroidism؟

📌 بسبب طريقتين:

🅰️ أولاً: Jod–Basedow Effect
إذا كانت الغدة تحتوي على nodular goitre أو Latent Graves’ disease:
الغدة تكون غير قابلة للتنظيم الذاتي.
زيادة اليود → تحفز الإفراز → Hyperthyroidism.

🅱️ ثانياً: Destructive Thyroiditis
Amiodarone يسبب أذية مباشرة لخلايا الغدة→تطلق الهرمونات المخزونة→Hyperthyroidism مؤقت.
