2) You should see all failed transactions through OS you've done - sort by 30 days.

3) All transactions that appear here are valid for OS Refunds under their TOS. Valid for up to last 30 days only

4) Open an OS support ticket and upload the csv file.

5) Wait and hopefully get a refund
