In a world-first medical achievement, doctors have restored sperm production in a previously infertile man by transplanting his own testicular tissue that had been cryopreserved when he was a 10-year-old boy.

The patient, who had undergone chemotherapy for sickle cell disease as a child, received the autologous transplant of his immature testicular tissue 16–17 years later as part of a clinical trial at the University Hospital Brussels (UZ Brussel) and Vrije Universiteit Brussel (VUB). The re-implanted grafts successfully resumed function, leading to complete spermatogenesis and the production of mature, motile sperm—particularly in the intra-testicular grafts.

While the patient will still need assisted reproductive techniques (such as ICSI) because the grafts are not connected to the natural sperm ducts, this breakthrough demonstrates that cryopreserved prepubertal testicular tissue can survive long-term freezing and reactivate spermatogonial stem cells in adulthood. It offers new hope for the thousands of childhood cancer and blood disorder survivors who had immature testicular tissue banked before gonadotoxic treatments.
