🔴Apicomplexa have a specialized apical complex that helps in cell penetration because the parasite is intracellular.

🔴Cryptosporidium was first discovered in primitive animals. Later, it was found that it can also infect humans. It mainly lives in the small intestine. However, under certain conditions related to weakened immunity, it may also be found in the stomach, appendix, colon, and rectum. Therefore, it is considered an opportunistic parasite.

🔴Cryptosporidium:
Cryptosporidium parvum is transmitted to both humans and animals; therefore, it is considered zoonotic.
Cryptosporidium hominis is considered less important than Cryptosporidium parvum because it is transmitted only between humans. Since it spreads from human to human only, it is described as anthroponotic.

🔴The oocysts contain sporozoites. They are small, oval in shape, and measure 15–20 micrometers in size.
🔴Cryptosporidium, like Giardia, is highly resistant to chlorine disinfection, which represents a major public health concern for water supplies. It can also survive at temperatures up to 60°C.
🔴Cryptosporidium lives in the small intestine; therefore, it is excreted in the stool.
🔴The oocyst is the infective stage and causes direct infection in humans. The oocyst contains four sporozoites.

🔴The life cycle of Cryptosporidium involves both sexual and asexual stages, ending with oocysts that are shed in the stool and can cause direct infection.
In the intestine, some oocysts may rupture and cause autoinfection, especially in immunocompromised individuals. This explains why infections can become chronic in immunocompromised patients, as thin-walled oocysts can rupture and lead to internal autoinfection.
Thick-walled oocysts, on the other hand, are excreted in the feces of an infected person or animal and are the primary source of environmental contamination. These can cause external infection.

🔴Cryptosporidium is widely distributed,If a patient is infected, you should ask about their occupation and other relevant exposures,The infection is considered a notifiable disease, meaning it must be reported to public health authorities.

🔴Acute symptoms of cryptosporidiosis include self-limiting watery diarrhea, which means it does not require treatment.

🔴Cryptosporidiosis in immunocompetent persons usually causes self-limiting watery diarrhea and does not spread to other organs.

🔴CHRONIC SYMPTOMS OF THE DISEASE
In severely immunocompromised patients, particularly those with advanced HIV/AIDS or certain genetic immunodeficiencies, the infection can spread beyond the intestines to other organs, such as:
The biliary tract, causing cholangitis
The lungs
It can even cause jaundice in some cas.
🔴To diagnose cryptosporidiosis, a stool sample is collected. A small amount of the stool is placed on a glass slide, and a drop of normal saline is added.

🔴When we stain the stool sample using Modified Ziehl–Neelsen (ZN) staining, this method is considered more accurate and more sensitive for the diagnosis of Cryptosporidium infection.

🔴the stool sample is reported as negative (free of oocysts), one sample alone is usually not sufficient to rule out infection.

However, a single sample may be enough if it has been properly examined using a stained method (such as Modified Ziehl–Neelsen staining), which increases the sensitivity of detection.

🔴Conjunctin B is usually used as supportive treatment in the eyes for cases of ocular cryptosporidiosis and was recently discovered, but it should be preceded by giving antivirals and increasing T-cell immunity.
🔴The oocyst in Cryptosporidium differs from the oocyst in Isospora belli because the Cryptosporidium oocyst contains sporozoites , while the Isospora belli oocyst contains two sporocysts, each of which has four sporozoites.

🔴In an immunocompromised patient with watery diarrhea, a CBC may show eosinophilia.
Eosinophils usually respond to parasite infections, but most intestinal parasites do not cause high eosinophil counts, so it is usually very mild
However, if high eosinophils are present, it may indicate chronic symptoms of Isosporiasis.

🔴Differential diagnosis means having the same symptoms but different causes.
Example:
A person has diarrhea, and the diarrhea is chronic. We did a stool test and found nothing abnormal.
The next step is to perform an endoscopy and take a biopsy from the small intestine.
If we find villous atrophy, possible causes include:
Celiac disease
Giardiasis
Other malabsorption syndromes
This process helps differentiate between diseases that present with similar chronic diarrhea but have different underlying causes.
🔴We cannot rely on a normal stool analysis for diagnosis.

🔴Treatment of Isosporiasis
Relapse: The symptoms may recur after initial treatment.

🔴We rely on staining and concentration techniques for diagnosis.
By concentration, we mean that when we perform centrifugation, each particle has a specific relative weight. The oocyst has a certain relative weight, so it settles in a specific layer. We can then collect that layer and stain it for microscopic examination.

🔴Entamoeba histolytica
-Enta means intestine.
-Amoeba refers to the portion used for movement (pseudopodia).
The word amoeba also means “changing”, because its shape is constantly changing.
🔴Entamoeba histolytica
The nucleus may be single or may contain two micronuclei.
Inside the nucleus there is a central karyosome, which contains DNA.
The chromatin is arranged peripherally along the nuclear envelope.
🔴Karyosome is the DNA-containing structure inside the nucleus.

It is a non-nuclear membrane–bound structure located within the nucleus.
🔴Entamoeba histolytica is considered the second leading cause of death among parasitic infections after Malaria.
About one-third of the world’s population is exposed to this parasite.
🔴Trophozoite of Entamoeba histolytica:
"We find RBCs in it only in the stage of invasion; white blood cells and food vacuoles containing RBCs are found. Because it is the pathogenic stage, the acute stage, and the growing stage, as differentiation for the reproductive stage occurs in it, and it is called the pathogenic stage.
🔴Entamoeba histolytica Both the trophozoite and the cyst stages are considered diagnostic stages.
🔴Entamoeba histolytica has a direct life cycle and does not require an intermediate host. Therefore, it is called a monoxenous parasite.
🔴One cyst gives 8 trophozoites.
🔴Diagnosis of Entamoeba histolytica
Direct Diagnosis: We aim to see the parasite itself (Trophozoite or Cyst).
Indirect Diagnosis: We aim to detect the antigen or the antibody.
Stool Examination: If nothing is found in the stool, it must be re-examined multiple times.
🔴In the large intestine, irritation of the cecal mucosa is called dysentery.
It is characterized by small amounts of stool accompanied by blood and mucus.
In contrast, irritation of the small intestine leads to faster intestinal movements and large-volume diarrhea.
🔴The Trophozoite is the pathogenic stage. In an amoebic abscess, we find the trophozoites in the peripheral area (the wall of the abscess), not in the central pus. This is because the trophozoites need living cells and tissue to feed on and survive, whereas the center contains only dead (necrotic) material.
🔴Amoebic Colitis
The patient may initially experience abdominal discomfort and constipation, followed by diarrhea.
Some symptoms may respond to treatment, while others do not respond, leading to alternating patterns.
The most common misdiagnosis for amoebic colitis is Irritable Bowel Syndrome .
🔴Extraintestinal amoebiasis the Right side of the liver most common site.
🔴Entamoeba coli is non-pathogenic.

🔴Naegleria fowleri trophozoites feed on CNS tissue.

🔴Naegleria fowleri does not have an intermediate host.

🔴The infective stage of Naegleria fowleri is the trophozoite.

🔴infection occurs and the amoeba reaches the brain without being detected in the CSF, it causes inflammation.
The complications and clinical presentation include:
Headache
Vomiting
Seizures
Later, it may progress to , coma, and sometimes paralysis.

🔴Examination of Naegleria fowleri infection:
Lumbar puncture: In the CSF, we may find high protein, low sugar, and high neutrophils, as well as the amoeboid form of trophozoites.
X-ray may also be performed as part of the evaluation.

🔴Naegleria fowleri enters the body through inhalation.
