🚨 CVE-2026-101041
The account recovery (password reset) functionality in the vulnerability-lookup web application contains a time-of-check-to-time-of-use (TOCTOU) race condition in the consumption of single-use recovery tokens. The original implementation verified the token nonce against the stored digest and then consumed (cleared) it in separate database operations. Two concurrent HTTP requests presenting the same valid recovery token could both pass the verification check before either transaction committed, allowing both to set their own password on the target account. The last transaction to commit overwrites the first, enabling an attacker who possesses a valid recovery token to replace the legitimate user's password with one of their choosing.

A secondary defect in the same endpoint (confirm_account) allowed a valid recovery link to be used to set an empty or trivially short password (e.g., three characters). The view handler performed only a manual equality comparison between the two password fields and never invoked the form's validation logic, bypassing the intended minimum-length and complexity constraints.

The affected component is the user account recovery endpoint (/user/confirm_account/<token>) and the associated token verification and consumption logic in the User model (website/models/user.py) and the view layer (website/web/views/user.py).

Apache Karaf's instance-management service (InstanceServiceImpl) builds the command line used to launch a child Karaf JVM by string concatenation, then executes it through /bin/sh (Unix) or cscript (Windows). The caller-supplied javaOpts value is spliced into that string unquoted. A javaOpts value containing shell metacharacters (;, |, `, $(...)) is interpreted by the shell instead of being passed to the JVM as an option, giving arbitrary OS command execution as the Karaf process user.


Reachable via the shell commands instance:create, instance:start, instance:restart, instance:change-opts, and the equivalent InstanceMBean JMX operations (createInstance, startInstance, changeJavaOpts, cloneInstance).

Mitigation  * Set karaf.secured.command.compulsory.roles=admin in etc/system.properties to close the fail-open gap for all unconfigured command scopes.
* Restrict which principals can reach instance:* commands and InstancesMBean via etc/users.properties role assignments.
* Treat javaOpts passed to instance:create/instance:start/instance:change-opts/InstancesMBean as untrusted input only from fully-trusted operators.

The method calls Class.forName(clazz).getConstructor().newInstance() where clazz is read directly from the CORE protocol wire buffer without type validation. An authenticated federation peer can send a FEDERATION_DOWNSTREAM_CONNECT packet with a crafted class name, causing the broker to load and instantiate arbitrary classes visible to the Artemis module classloader. Static initializers (<clinit>) and no-argument constructors (<init>()) execute as side effects before the type cast, enabling denial of service via system-property poisoning, out-of-memory conditions via classloading, or broker state manipulation.

FreePBX is an open source IP PBX. Prior to version 17.0.9, authenticated users who are authorized to access the GraphQL api module interface of FreePBX are able to execute arbitrary shell commands.
