الـ septic shock من أبرز مسببات الوفاة للمرضى عدنا،
ولهذا هنا كون نكتشف الـ sepsis من وكت
قبل لا توصل للشوك …
الخطأ اللي نرتكبه هو اعتمادنا على صعود الـ WBC والصخونة حتى نگول sepsis،
والواقع أنه هاي مو ضمن الكرايتيريا …
ولهذا هواي sepsis ما ندگنزهم لأن WBC نورمل، والعكس صحيح …
أولاً: شلون نشك أكو sepsis؟
اذا شكيت أكو إنفكشن + ٢ من ذني:
(RR > 22, SBP < 100, GCS < 15)
بأوكسفورد يگول بس وحدة منهن كافي.
ثانياً: شنو ندز؟
١- ندوّر على مصدر الإنفكشن: أشعة صدر، تحليل إدرار
٢- نشوف إذا أكو end organ damage: فندز مثلاً RFT, LFT
٣- ندز ABG علمود الـ Lactate، وCBC، وblood C&S
ثالثاً: شلون نعالج؟
بمجرد الشك بوجود sepsis ابدي العلاج، لأن كل ساعة تأخير تزيد المورتالتي ١٠٪ حسب ديفدسن، والعلاج هو:
١- أوكسجين، ويكون high flow
٢- نورمل سلين ٢٠ مل/كغم + نورادرينالين إذا بقى هايپو
٣- IV Ab (Tazo or mero + vanco)
نعيد الـ lactate وره ساعتين حتى نشوف الاستجابة.

A 53 years old man presented to the ward with 5 days hx of FBM And vomiting..
Patient later developed Vt ( torsade de pointe) at night and referred to ccu where he returned by DC shock and given kcl and mg sulfate
And when i took this case in the morning the patient was Dloc with multiple seizures attack that stopped with diazepam
Ix as shown in the pic
Further management with kcl and mg sulfate continued with cover up antibiotics
What do you think about such case?

An old age female presented with sob and palpitations
On examination
Clear chest
spo2 =98
PR 160
Bp 110/70 mmhg
RBS CbC , S troponin all was normal
ECG was done as shown below in comments
Adenosin 6 mg was given pulse reduced from 154 to 85 bpm
But few minutes later returned to 155 bpm
Another 12 mg of adenosine was given and reduced to 75 bpm but again few minutes later became 163bpm
A shoot of NS was giver then isoptine 5 mg directly the patient pulse become 85 bpm and this time didn’t raised upove normal😂💔
On examination
Clear chest
spo2 =98
PR 160
Bp 110/70 mmhg
RBS CbC , S troponin all was normal
ECG was done as shown below in comments

Patient looked pale and fatigue
No BP or pulse was detected
ECG was done and show the pic below
A shot of NS was given then we got a bp of 80/60 Then we delivered a single synchronized DC shock of 100J to him

بعد ما اخذ الانتي اسكيميا تم اعطاءه اكتليز و حسب البروتوكول
بعد ٦ ساعات patient developed disturbed level of consciousness, mouth deviation and lt side weakness
CT shows ICH and IVH !!
صحيح هو واحد من السايد افكت للاكتليز ولكن هالشي كلشش رير ونسبته قليلة
هل السبب انو جان عنده كونترا اندكيشن ؟ او الهيمورج مو بسبب الاكتليز انما كوفد ملام ؟ لان السي تي بين اكو انفلتريشن باللنك ايضاً يمشي وي كوفد
