This is a case of Ocular Flutter in Serotonin Syndrome which can occur when patient increases the dose of certain medications or start taking a new drug ( MAOI / SSRI / SNRI )

بصراحة كومة جاي تصير ممارسات خاطئة اتجاه كرصة الافعى او العقرب
يعني حتى ع مستوى ممرضين و اطباء ماعرف ليش جاي يشجعون ع استخدام التورنيكا يشدون بيها الطرف المكروص او تشريح مكان الكرصة علساس يطلعون الدم المسموم من الجرح او حتى مص الجرح
يعني هاي كلها ممارسات خاطئة تزود ال limb ischemia and increase risk of wound infection
كل الي يحتاجه الشخص هو فقط تثبيت الطرف الي تعرض للكرصة نقلل الحركة قدر الامكان و ع اقرب مستشفى حتى ياخذ العلاج المناسب والي هو غالبا
-ABC and iv access in non venomated limb
- wound care and ATS
- Anti venom therapy accordingly
- monitoring

بما انه هالايام هواي يجون سكوربين بايت هذا مخطط لطيف للمنجمنت
بصورة عامة اهم شي الضماد و البين منجمنت الي هو سستمك مثل فولتارين او بروفين
و لوكال زايلو كلش مهم خطية يجون يكلولك عندي سعير برجلي او ايدي (حسب مكان الكرصة)
و مننسى الATS
يبقى الانتي فينوم حسب ال grade مالته مثل متشوفون بالصورة

لذلك المفروض ننطي فقط ٢٥٠-٥٠٠ مل فلود و نبلش فازوبرسر مثل النور ادرينالين او دوبيوتامين …

A teenager presented with sudden loss of consciousness.. family reported he had one week hx of diarrhea otherwise normal
On examination he was vitally stable but very feverish with bilateral fixed dilated pupils!!
GCS 3-4
Lab Ix and CT Brain was normal
No neck stiffness
Babenski negative
And I noticed some scars all over his both hands probably from suicidal attempts (family said it’s from work! )
At that time i was thinking about drug abuse or Encephalitis as a possible cause after all these findings what do you think ?
After digging in hx the family reported that the pt had hx of burn 2 months prior this admission and he was taking antihistamines from time to time!
And I believe there is something more hidden about drug hx
