Myocarditis:~
The myocardium is the middle layer of the cardiac wall
It consists of cardiac muscle(myocytes)
⚪️myocarditis : is inflammation of the myocardium it can be caused by many causes or it could be idiopathic.
🔵Infective myocarditis:
Caused by infection by:
~virus: which is the most common type of myocarditis, many viruses can cause myocarditis but the most common are enteroviruses especially Coxsackie A&B viruses
~Protozoan: like
~toxoplasma gondii: that infect
human from cats, usually in immunocompromised people.
~tryponosoma cruzi :in chagas myocarditis : the parasite invades the myocytes and cause lymphocytic and neutrophilic infiltrate.
~Trichinosis: most common helminthic (ديدان) disease with cardiac involvement
~Bacteria:
~spirochete Borrelia burgdorferi:as rare complication of lyme disease
🔵Non infective myocarditis: could be caused by:
~Autoimmune disease : like systematic lupus erythromatous.
~Hypersensitivity by drugs: which is a very rare complication, cause eosinophilic infiltration and has a poor prognosis (the patient may die😢)
🔵 Idiopathic myocarditis(the cause is unknown) : either
~1-deffuse myocarditis : which is more common and have infiltration of many types of cells (lymphocyte, macrophage, neutrophile..etc)
~2-giant cell myocarditis: granuloma, multi nucleated giant cell
Has bad prognosis, necrosis may occur
▪️Thrombosis is common in idiopathic myocarditis.
🔊Myocarditis could be acute or chronic
🔊It could heal on its own or progress into cardiac failure and cardiomyopathy.
Diagnosis:~
🔵Serological and molecular methods: to identify the Infective agent if it is infective myocarditis.
🔵ECG: in the case of heart failure and arrhythmia.
🔵Markers of myocyte damage(cardiac enzymes) :like treponin
🔵Biobsy: gold standard (but its difficult to take biobsy from the HEART)
⬛️Clinical manifestation:
Myocarditis could be:
$Asymptomatic
$cause HF
$cause sudden death 😔
~consider in young patients (especially children) who doesn't have prevous cardiac complication.
~upper respirotary symptoms like dyspnea
~in sudden onset cardiac failure and arrhythmia
~cardiac complications after systemic illness.
⬛️Treatment :
Supportive by :ionotropes(which increase the contractility thus strengthen the heart beat)
Diuretics :to decrease the workload of the heart (diuretics cause urination by that we decrease the amount of blood then the heart won't work hard to pump the blood)
