🚫Fluoroquinolones have been associated with disabling and potentially irreversible serious adverse reactions that have occurred together, including tendinitis and tendon rupture, peripheral neuropathy, and CNS effects.

📮β-Blockers with intrinsic sympathomimetic activity (eg, acebutolol, pindolol, and penbutolol) have partial β-agonist effects 
and cause lesser reductions in heart rate at rest. As a result, 
β-blockers with intrinsic sympathomimetic activity may produce 
lesser reductions in myocardial oxygen demand and should be 
avoided in patients with SIHD.

🅾️Antibiotic Recommendations for
Acute Appendicitis

📮CASS recommends
✔️cefTRIAXone 2,000 mg IV q24h PLUS
✔️metroNIDAZOLE 1,500
mg IV q24h (with pharmacist managed IV-to-PO conversion to metroNIDAZOLE 500 mg
PO q8h due to concerns regarding oral tolerability) as first line therapy for adult patients
with acute appendicitis.
– We are not concerned about excess neurotoxicity associated with the use of
metronidazole in this order set. Based on a review of the literature, we feel
metronidazole-induced acute neurotoxicity to be rare and reversible, and not more
prevalent or toxic than alternative regimens.

✅ We recommend avoiding long-term use of metronidazole (> 3 weeks) when possible
given the risk of peripheral neuropathy, which has in some cases been very slow
to resolve.
✔️ In patients with cephalosporin allergies, we recommend ertapenem 1 g IV q24h
✔️CASS would be supportive of either amoxicillin-clavulante 875 mg PO BID or
levofloxacin 500 mg PO daily PLUS metroNIDAZOLE 500 mg PO q8h (penicillin
allergy) as oral step-down regimens.

✅In AF

✔️ direct-acting oral anticoagulants are preferred to vitamin K antagonists because of their reduced bleeding risk.

✔️Warfarin remains the drug of choice in patients with mechanical heart valves and in those
with moderate to severe mitral stenosis.
✔️If warfarin is used for triple therapy, it is reasonable to maintain the international normalized ratio (INR) at the lower end of the therapeutic range (i.e., 2–2.5).
✅ Clopidogrel is the preferred P2Y12 inhibitor, though ticagrelor may be reasonable in patients at
high ischemic/thrombotic and low bleeding risks (i.e., avoid prasugrel).
✔️ Discontinue antiplatelet therapy at 1 year in most patients.
✔️ However, the same considerations apply for shortening the duration (i.e., 6 months in
those with high bleeding risk) and extending the duration (i.e., more than 12 months in those at high thrombotic risk with low bleeding risk) as for those without AF.
✅ Proton pump inhibitors should be considered for those with a history of GI bleeding (and is reasonable in those without a known history of GI bleeding) who need triple antithrombotic therapy..

✅There is evidence that angiotensin-converting enzyme inhibitors (ACEIs) and
angiotensin receptor blockers (ARBs) reduce the risk of stroke in diabetics, with or
without hypertension.

#معلومة
✅ use of oral hypoglycemics may reduce the risk of
stroke through mechanisms other than glycemic control. However, strict control of
blood glucose did not reduce the risk of stroke over the course of 9 years in one study.

✅It is known that HMG-CoA reductase
inhibitors have anti-inflammatory activity that may influence the development of
atherosclerotic plaques and cerebral ischemic processes.
✅ Because of these effects of HMG-CoA reductase inhibitors, these drugs should be started for primary prevention of ischemic stroke and TIA even in patients who do not have dyslipidemia
if the estimated 10-year risk of cardiovascular events is high, as recommended in the
2013 “ACC/AHA guideline ..

🅾️The treatment of Ischemic stroke in atrial fibrillation patients..

✅If Acute treatment:
Alteplase 0.9 mg/kg IV (maximum dose 90 mg); 10% as IV bolus,
remainder infused over 1 hour in select patients within 4.5 hours of
onset.

✅Secondary prevention:
✔️Warfarin (INR 2–3) or
Apixaban 5 mg twice daily; decrease to 2.5 mg twice daily if patient has
at least two of the following: age ≥ 80 years, weight ≤ 60 kg (132 lb)
or serum creatinine ≥ 1.5 mg/dL (133 μmol/L) or
Rivaroxaban 20 mg daily with the evening meal; decrease to 15 mg daily
with the evening meal if CrCl 15–50 mL/min (0.25–0.83 mL/s)

✔️Bp control
If Systolic BP ≥ 220 mm Hg or diastolic
BP 121–140 mm Hg
Labetalol 10–20 mg IV over 1–2 minutes (may repeat every 10–20 minutes; maximum dose 300 mg)
or nicardipine IV 5 mg/hour (titrate up by 2.5 mg/hour every 5–15 minutes; maximum dose
15 mg/hour; titrate to response) or clevidipine 1–2 mg/hour IV (titrate by doubling the dose every
2–5 minutes; maximum dose 21 mg/hour; titrate to response)
If Diastolic BP > 140 mm Hg ,Nitroprusside 0.3–0.5 mcg/kg/min titrated by 0.5 mcg/kg/min to response; maximum dose 10 mcg/kg/min

✅The use of a statin
Treatment recommendations are based on current guidelines for treatment of blood cholesterol..

الله يلعن أبوها عروبة ويلعنها نخوة، أخواننا يموتوا بالآلاف من أقل قضية إسلامية عالمية، ونحن اصحابنا مهتمّين بالتفاهات، قبح الله وجوهكم، اجل شيء ممكن نقدمه أنه ننقل جرائم أولاد الحرم وما يرتكبوه بالشعب الفلسطيني للناس والعالم ويعرفوا الجزء الصادق من القصة.. لأنه إعلامهم يحاول أن يضلل عن شعوب الغرب و متهم حماس بأنها هي من تذبـ.ـح الأطفال..
لذلك حاولوا تجاهدوا بالكلمة وهذا أكثر واجب ممكن نستطيع نشارك به ،(وما النصر إلا من عند الله)

مش وقت الخبالات هذه الأيام والله..! 💥

دواء "Nimodipine" الذي يوسع الأوعية الدموية في الدماغ و يستخدم لمنع حصول (ischemia) في الدماغ بعد أنواع معينة من نزيف الدماغ ..

هل الطعام يقلل امتصاصه ؟؟ خصوصا أنه يجب تناوله مرة كل 4 ساعات (يعني 6 مرات يوميا) .. فهل لازم تكون المعدة فاضية في كل مرة ؟؟


📚 الجواب 👇👇👇

لازم نشوف شكله الصيدلاني:

1️⃣ إن كان على شكل "Capsules" تحوي داخلها مادة نيموديبين على شكل gel .. فيجب تناوله على معدة فارغة (قبل الطعام بساعة أو بعد الطعام بساعتين) ..

لأن الطعام يقلل امتصاص الدواء بنسبة 40% (يخفض التوافر الحيوي من 13% إلى 8%) !!

طبعا الكبسولات مشهورة في بعض دول العالم ..


2️⃣ أما إن كان على شكل "Tablets" مثل صنف Nimotop الشهير .. فالطعام يبطئ امتصاص الدواء لكنه لا يقلل امتصاصه (ويعزى ذلك إلى اختلاف الexcepients) .. لذلك يمكن تناوله على معدة فارغة أو ممتلئة ..

هذا النوع هو المشهور في اليمن .. وهو الأسهل في الاستخدام خصوصا أن الNimodipine يتم استخدامه 6 مرات يوميا -كما سبق- !!
