💢مُلاحظات العملي:-

-الدكتور مُحمَّد الشهاري.
-اليوم: الإثنين 19.8.2024.
-مُستَشفى الثورة.

💢بدأ الدكتور بالتذكير السريع عن عناوين ال swelling الذي أخذناه في أول أسبوع مع كل جروبات الجراحة، تقريباً عتلاقوها بِالمُلخصات الذي فوق👆🏻 بحكم أنه قد أخذناه مع كل الجروبات بِ أول أسبوع الدكتور ذكره ك َعناوين فقط، وبدأ بِ أخذ ال History منّ الطلاب، طبعاً الدكتور يهتم جداً ويركز بال History مع تفاصيله الصغيرة، ومش كل مايقوله المريض نكتبه وخلاص، لابد أنه نحلله ونرتبه ونحلل معطياته ونبذل جهد فيه، لذلك إقرؤوا الملخصات الذي فوق لاازم، وهذه إضافات فقط وأهم مُلاحظات الذي ركز الدكتور عليها في ال History :

🔹ال History ينقسم لنوعين:
1. Translated history :
تأخذه وتترجمه على ماقاله المريض وخلاص .
2. Investigated history :
تحلله وترتبه وتحلل مُعطياته ( بِالمختصر تبذل جهد فيه ).

💢Note ➡️ You can't find painful neck swelling for 2 years !!
ماتركب مع ال Thyroid أنه ال painful ويجي بعدها swelling ماتركب أبدااً !!! أول شيء يبدأ swelling بعدين pain .

🔹Example:
-The condition started 2 years ago as swelling in the anterior part of the neck, this swelling is insidious in onset, progressive in course, slowly in this period, but the last 2 months become rapidly progressive ,and associated with pain in these 2 months ( or say this swelling was painless ,but last 2 months become painful and increase in the size ).

🔹Non inflammatory ➡️ insidious in onset ➡️ discovered accidentally.

🔹Thyroid ➡️ is indolent tumor ➡️ A type of cancer that grows slowly .
( ماعدا ال growth في ال Anaplastic يكون مُمكن خلال شهر أو شهرين ).
-وماتركب thyroid abscess & infection لمدة شهرين !!


🔹Thyroid cancer can be:
-Differentiated ➡️( Papillary, follicular, medullary, poorly differentiated).
-Undifferentiated ➡️ Anaplastic.

🔹لما يقول المريض أنه الشكوى مضايقه له  ( لكنّ مايأخذ مهدئات) هذا مش pain لكنّ المريض attention to something else مثل أنه يشتي إهتمام وخلااص ، لذلك بأي شكوى يذكرها المريض لااااااازم نحلل الشكوى ونسأل ونركب ونرتب كلام وشكاوي المريض ونبذل الجهد  بأخذ ال History 👍🏻 .
🔹The direct effects of interferon-alpha, cytokines, interleukin-1beta, and tumor necrosis factor-alpha on glucose-sensitive neurons in two specific regions of the hypothalamus could cause profound changes in eating patterns in animals and humans.
الورم يحصل releasing in inflammatory markers بالجسم بشكل عالي(الجسم يستجيب للورم بشكل مش طبيعي).

🔹Anorexia :
إذا ذكرها المريض نسأله عدة أسئلة عنها، كم لك ماتأكل؟ من متى؟ وهل وجبات معينة ماتأخذها؟ وزنك نقص؟ الوزن الذي نقص لازم يكون نسبته أكثر من 10% في ستة أشهر .. وأسئلة تدور عن هذا الشيء بحيث تتأكد، بِالمُختصر مش كل مَارمى لك المريض تكتبه وخلاص .. لااااتكون سهل الصيّد 👍🏻.

🔹Most patients with superficial invasion of the trachea are asymptomatic. Symptoms will appear when they experience intraluminal invasion. Common symptoms include hemoptysis, stridor, dyspnea and airway hemorrhage.

🔹في ال Thyroid بحالة ال Benign مستحيّل نلاقي dyspnea، لو تكبر مهما تكبر ماتعمل dyspnea، يعني بالمختصر لما نتكلم على المُعطيات، لازم تكون related to the history يعني نعمله بِ إحترافيه ..

🔹Tracheomalacia :
هي لفظ مُمكن يكون بعد ماتشيل ال Thyroid.
🔹Tracheomalacia presents as an immediate airway compromise, presenting as stridor after extubation in a patient who has undergone thyroidectomy.

🔹بِالنهاية الدكتور شرح ال examination موجودة بِالمُلخص الذي فوق👆🏻..

_All HTN drugs still use during operation
Except diuretic stop it.

_ Metformin stop during operation..

_ Aspirin/
In small surgery continues use


_Pt most be stop smoking 3 months before major operation..

_DM/
Before surgery most to be less than 200 mg
او في التراكمي اقل من
(8.5)
.

_HTN/
before major surgery most to be
Systolic less than 160mmhg
Diastolic less than 100 ,110 mmhg

_pt most be fasting 6_8 hours before operation


_ the early warning sgin in pt came to ER is tachypnea..

تكلم الدكتور .How to examine L.L ؟

( يفضل نبدأ بذكر الve finding+ وبعدين ال -ve finding ونفحص ال anterior and posterior aspect of the L.L )

☆ By inspection:
● Asymetry or not بالمقارنة مع other LL

● Comment if there is :
edma, scars, dicoloration of skin, pigmentation, slough of the skin, ulcer, dilated veins, wound, discharge, trophic changes = [ loss of hair, dry scaly shiny skin, brittle nails, interdigital infection ], amputated toes or not.

Scar ( نحدد الsite , healing by 1ry or 2ry intention)

Ulcer ( number, site, shape, size, and describe the edge, margin, floor, and if there is discharge )


Burger's angle & test نعملها مع inspection
( اذا ماظهر معانا الangle مافيش داعي نكمل الtest )

☆ By palpation: ( start with normal then abnormal L. L.)

Temperature, pulse, capillary refill اهم ٣ أشياء
وبعدين البقية
tenderness
Edma ( detect the level, pitting or not, degree of edma)
Examine if there is ulcer ( mobility of the base or not in 2 prependicular lines, squeeze if there discharge or not)
Neurological examination ( motor and sensory examination )



☆ مالفرق بين ال cellulitis & erysiples & Necrotizing fascitis ?

Erysiples has raised elevated red skin, and well-defined edge.

Cellulitis has signs of inflammation, hottness, redness and edma and flat skin without discharge and has ill defined edge.


NF has discoloration and diacharge.

● Note : Compartment syndrome mostly occur in leg and pt. came complaining of severe pain.

-It is between the two imagine line of Mid Clavicles line's of both sides

- types (parts)
Upper part
Lower part ➡️ subdivided to Ant., Middle and Post.


♦️ The length of oesophagus:

- Anatomical length ➡️ 25 Cm
- Clinical length ➡️ 40 Cm from central incisors to the stomach...



♦️ Pre_malignant of oesophagus:
- Frist Barrett's oesophagus
- second Corrosive strictures of oesophagus
- Plummer Vision Syndrome
- Achalasia of Cardia...
