💢 طريقة أخذ الـ Examination

1- Wash your hand .
2- precede your self .
3- 30 to 45 deggre .
تكون مرتفع عن المريض
4- Exposure the area

🔺First ...
Inspection we see 3 things :-
1- Risk factor of embolism :
A. Obesity
B. Old age
C. Pregnant

2- Clinical sings:-
A. Homan's sign dorsal flexion of the leg lead to the pain in the calf muscle
B. Mosts sing squizeing!!!
3 اشوف ماذا يوجد بجانب المريض كالعكاز مثلاً أو أي شيء أخر ...

🔺2nd ...
Palpation :-

1- Temperature بتكون مرتفع
2- Pulse and blood pressure
  اشوفه لأنه قد يكون مافي pulse مثلاً في
Atherosclerosis and renal disease
, Thoracic outlet syndrome and tumor mass
اشوف الضغط لأنه قد يكون المريض عنده Hypertension وهذا يسبب
Endothelial injury.

3- Tenderness
4- Swelling and redness and oedema
5- Pain

🔺 3rd:-
Auscultation "ليست مهمه"

🔴 Signs of D.V.T patients:-

1- Pain
2- Swelling
3- Redness

🔵 Causes of thromboembolism:-

1- Endothelial injury :

A- Hypertension and Smoking.
B- Trauma and Endotoxin.

2- Circulatory stasis :

A. Varicose veins and anyorysm
B. Arythematis and immobility

3 Hypercoagulation :

A. Decrease antithrombin "III" and proteins C@S.
B. Atrial fibralation and Contraceptive.

🔵 Predisposing factors of Thrombosis:-

T➡️ Trauma.               
H ➡️ Hormonal. e.g 
contraceptives.           
R ➡️ Road Traffic accident.    
O ➡️ Obesity and Old age.
M ➡️ Malignancy.
B ➡️ Blood disorder and Polycythemia.
O ➡️ Orthopedic surgery and Operations.
S ➡️Stroke.
I➡️ Mobilization.
S➡️ Spleenectomy.

🔻Phlegmasia alba dolens:
تكون
Pain and oedema and Pale skin rash.

🔻Phlegmasia cerulea dolens:
تكون
Pain and oedema and cyanosis

🔵 ttt :-

1- Elevate the leg to relate symptoms .
2- Elastic compression of the leg 🦵
3- Analgesic .
4 -Heparin or warfarin .
5- Thrombolysis .
6- Venous thrombectomy .
7- Prevention of ather embolism by filter .

🔵 Investigations:-
        by Doppler or Duplex ultrasound scan.

💢 Anaesthesia 💢

🔺 Propofol ... It's the better one used as anaesthesia fro all the healthy patients.
⏰ The duration of 15 - 20 minutes.
🚫 It's C.I at ... Shocks and Hypotension and Dehydrations patients.

🔺The better one anaesthesia for the patients with hypotensions ... Is
ketamine


🔺 Amitriptyline is the better drug for treatment of Backache after spinal anaesthesia.

🔺 Fentanyl ... It's the better and safer as analgesia for patients after anaesthesia.
- Dose 2 ug/ka to maximum dose 150 ug.
- It is more strong of morphine by 100 time.

🔺 Halothane ... should be the duration between the first use and second uses 26 days as minimal interval


🔺 Isoflurane ... it is the most effective and safer anaesthesia for all patients

🔺 The site of anaesthesia for gallbladder operations ... is between
L7 - L8
- Dose = 2 cc of marcine

🔺Pethidine ... it is not used the now due to has high Addiction.
