The diagnosis is LEAD POISONING

BUT THE CHOICE FOR CHELATION THERAPHY IS NOT penicillamine (it was a previous choice but now not recommended because of its side effects)

the following are the chelation of choice for lead poisoning
- Succimer
- ⁠calcium disodium edetate (CaNa2EDTA
- ⁠Dimercaprol



So what’s the answer?
Iron supplements
Why? Because of Concurrent iron deficiency anemia with lead poisoning

Wilms Tumor

Explanation below 👇
طيب هاذي الاسئلة فيها فتي كثير فخليني اختصرها
Wilms tumor (Nephroblastoma) vs abdominal Neuroblastoma

Based on UTD:
Presentation
- BOTH can present with Hypertension
- Hematuria in wilms (because of it’s origin from the kidney…unlike neuro which is from adrenal medually which secretes catecholamines in urine)

Age: (Based on Radiopedia and Nelson)
- Neuro: below 2yo (Nelson: median age 17 months)
- Wilms: Above 2yo (Nelson: median age 31 months)

Regarding crossing the midline (Radiopedia)
- when he says in the question “ Going toward the midline” it DOES NOT mean it has crossed it!!! So don’t depend on this feature
- Neuroblastoma: it “May” cross the midline (so in the early stage it still yet to cross…and you can mistake it with Wilms)
- Wilms; does not cross

Associated syndrome (Nelson)
- neuro:
- opsoclonus myoclonus syndrome
- Proptosis (infiltration of eye muscle)
- Wilms: WAGR syndrome







So in this question why is it Wilms?
The age above 2 years is the main factor
Since HTN can occur in both tumors
Crossing of midline is yet to happen so you can’t say it is neuroblastoma

Option B Face involvement in general is more common in infants (another clue that excluded option B is perioral dermatitis is usually blisters and crusty and NOT plaques)

According to age and type of the lesion (plaque)
it’s more common in flexural areas
So it’s either A OR C

Infant 4 month medically free came with subcostal retraction dyspnea fever cough sneezing ,they gave O2 and saturation still 80 peripheral pulses weak and pale + pt had diffuse bilateral crackles and bailateral decreased air enetry+ his Bp is 110/70. Cyanosis and sacral edema Not mentioned, what’s the management ??
A-Diuretic
B- IV fluid
C- intubation and mechanical ventilation

A- Bone marrow aspiration

Dx: Hodgkin lymphoma (🔑keyword; unilateral Swelling in neck)

Metastasis to Bonemarrow caused Thrombocytopenia (which presented as bruising)

Case of clear turner (no doubt), hight in the 5th percentile and wight in the 90th
- constitutional stature
- endocrine stature
- syndromic short stature


She was having webbed neck and she was short




SMLE Radiance: I think this question has a bad recall….since Turner syndrome SHOULD NOT have obesity (over weight defined as percentile above 85th to 95th, like in this case it is 90th)

Just wanted to make it clear that this pattern (where the baby is short and obese …ACTUALLY INDICATES ENDOCRINE SHORT STATURE)

Having anaphylaxis from the same vaccine is the only true contraindication
