This was SANT

 Sclerosing angiomatoid nodular transformation (SANT)


Splenic lesions have non specific imaging findings and difficult to diagnose on imaging.
SANT may have hemosiderin which will appear hyperintense on in phase images like this case.

Imaging features

Single splenic mass without additional findings of lymphadenopathy

Ultrasound: Well-circumscribed, hypoechoic mass

CT: Low-attenuation heterogeneous mass with peripheral and progressive enhancement

MRI:
Heterogeneous low signal intensity on T1- and T2-weighted images; likely from hemosiderin deposition
Decrease in signal intensity on in-phase compared with out-of-phase images on chemical shift imaging due to hemosiderin content: This can differentiate SANT from other solitary solid splenic lesions.
Early peripheral enhancement progressing centripetally in a radial pattern; a "spoke wheel" pattern: This corresponds to histopathology to concentrated angiomatoid nodules around the periphery that radiate between the fibrous scar.

SPECT-CT: Scintigraphically cold on technetium-99m sulfur colloid imaging, likely due to the absence of reticuloendothelial cells
