They submit your stolen credentials to the real site in real time. The real site sends you a real text from a real number. Simultaneously the fake page shows you a box asking for that code. You enter it. They enter it. The thirty second window closes. They are inside your account.
Your two factor authentication worked perfectly. It just authenticated the wrong person.
You did not click a suspicious link. You did not download anything. You did not ignore a warning. You did everything right. And you still lost.
This attack has a documented name. Microsoft and security researchers call it AiTM Adversary in The Middle. Microsoft’s own threat intelligence team has published reports on AiTM campaigns targeting millions of accounts. The attacker is not hacking the platform. They are using you as the bridge. You are the exploit.
The fake page looks identical to the real one. Same logo. Same layout. Same button. Pixel for pixel. You cannot tell them apart.
The only way to catch it is the URL bar. And most people never look.
Real 2FA only protects you if you initiated the login. A code arriving when you did not just log in somewhere is not a verification. It is a receipt. Someone just tried to use your account.
If you ever receive an authentication code you did not request, do not enter it anywhere. Call the company directly. Then switch to a hardware security key if supported. Standard SMS 2FA cannot stop this attack. A hardware key can.

If you ever receive an authentication code you did not request, do not enter it anywhere. Call the company directly. Then switch to a hardware security key if supported. Standard SMS 2FA cannot stop this attack. A hardware key can.
