A- Stop paroxetine because of fetal malformation
B- Continue paroxetine and control her depression

Abdulrahman: The answer is B since she has severe depression with suicidal attempts. Here is the explanation as to why we shouldn’t stop or switch paroxetine. UTD: If a medication was taken during conception, then the fetus has already been exposed and the medication should remain the same unless otherwise indicated by patient preference.

Conservative treatment of thrombosed hemorrhoids is usually sufficient. Exceptions include those with severe pain and thrombosed external hemorrhoids, Conservative with pain medications and stool softeners. Dr. Abeidi: Thrombosed piles. We will do hemorrhoidectomy later. The recommendation for surgical treatment if symptoms are less than 24 hours is for evacuation of the hematoma (not for hemorrhoidectomy).
