Rheumatic Fever Prophylaxis
Recent MI. When to do cholecystectomy?
- If bare-metal stent PCI —> after 6 weeks
- If drug-eluting stent —> after 12 months

Open or laparoscopic?
- HF or low Ejection Fraction (below 50%) —> open
- Only MI —> laparoscopic
Management of Barret's esophagus:

🟪 Barrett alone (metaplasia):
PPIs and rescope every 2–3 years.

🟪 Low-grade dysplasia:
PPIs and rescope every 6–12 months.

🟪 High-grade dysplasia:
Ablation with endoscopy: photodynamic therapy, radiofrequency ablation, endoscopic mucosal resection.
CTG findings:
MgSo4 -> decreased variability
Oxytocin & cord compression-> variable deceleration
Epidural -> prolonged deceleration
Head position -> Early deceleration
Normal -> Acceleration

What will decrease mortality in COPD patient? 🧐

1- Smoking cessation (if it is in choices it is always the best answer)
2- Vaccination (Pneumococcal and influanza vaccine)
3- Home oxygen therapy (only if indicated, don’t choose it unless indicated!)

Indications for long term home oxygen therapy in COPD patient:-
1- PaO2 in ABG (out of time of acute attack) less than 55
2- PaO2 55-59 + one of the following: dependent edema (Cor pulmonale), polycythemia (Hct > 56%), pulmonary hypertension, Nocturnal hypoxemia

Normal pressure hydrocephalus: dementia + unsteady gait + urinary incontinence + CT might show ventricular enlargement and hypo intensities.

Frontotemporal: dementia + personality changes
