Acute cholangitis is an acute bacterial infection of the biliary tree, most often triggered by obstruction of the common bile duct, commonly due to gallstones, strictures, or tumors.

The obstruction increases intraductal pressure, allowing bacteria to ascend from the duodenum and proliferate. Clinically, it classically presents with
- Charcot’s triad: fever, jaundice, and right-upper-quadrant pain.
- In more severe cases, Reynolds’ pentad (triad + hypotension + altered mental status) indicates life-threatening sepsis.
