EHRs have evolved from initial development in 1992 to today, facing challenges but enabling precision medicine. The paper traces EHR history, issues, and future potential for international interoperable standards to foster a learning health system. In the early days of Electronic Health Records (EHRs), the focus was primarily on automating administrative tasks like claims processing and document capture through image scanning. These innovations significantly reduced the time spent on filing, chart retrieval, and other manual processes. As EHRs evolved, they began to integrate clinical functions, enabling physicians to access a wide range of patient information, including notes, orders, lab results, and radiological studies, all from a single workstation.
