Dear Sir or Madam,

The reason why I am writing this letter to you is that I am going to explain why I need to resign as well as I want to tell you my thanks for working together so far.

I have been working in your company for more than 5 years, yet I am going to leave my job because of my health problem. I went to hospital last week, and the doctor said I do not have to work anymore due to a negative impact of my health.

Therefore, I decided to resign my job as soon as possible. I wrote to you the application about my resignation two days ago. I cannot work from upcoming the week, and I am not sure when I am able to come back to work again.

As I mentioned before you gave me a job five years ago, and during the time we have been working together with you and my colleague. Thus, I have to say my gratitude to you and my co-workers.

I hope you will understand my situation and accept my application as soon as possible.

Best regards,
Bahodir To’qsonboyev.
