Position Summary
The Quality Assurance Pharmacist is responsible for ensuring full compliance with all Medication Management Policies across the hospital. The role includes active participation in the Pharmacy & Therapeutics Committee, conducting audits, ensuring adherence to medication safety standards, and training healthcare staff on medication policies.

Full Responsibilities
1. Medication Management Policy Compliance:
🟡 Follow and monitor all medication-related policies including:
🟩 Medication Storage Policy
🟩 High Alert Medication Policy
🟩 Controlled Drugs Policy
🟩 Drug Return Policy
🟩 Drug Purchasing Policy
🟩 Drug Dispensing Policy
🟩 Drug Administration Policy
🟩 Antibiotic Prophylaxis Policy
🟩 Antimicrobial Stewardship Program
🟩 Adverse Drug Reaction (Pharmacovigilance) Policy
🟩 Medication Reconciliation Policy
🟩 Crash Cart & Emergency Medications Policy
🟩 Sterile Compounding Policy
🟩 MDV (Multi-dose Vials) Use Policy
🟩 Essential Drug List Policy
🟩 Drug Labeling & Identification Policy
🟩 Medication Error Reporting Policy
🟩 Medication Evaluation & Outcome Assessment Policy
🟩 Drug Storage & Temperature Monitoring Policy

2. Training & Education:
🟡 Train physicians, nurses, and pharmacists on:

3. Committee Participation (Pharmacy & Therapeutics Committee):
🟡 Attend monthly meetings.
🟡 Provide reports on medication compliance, audits, and findings.
🟡 Participate in reviewing and updating medication policies.
🟡 Assist in designing and approving treatment protocols.
🟡 Contribute to essential drug list and drug formulary updates.
🟡 Monitor adverse drug reactions and medication errors.

4. Audits & Inspections:
🟡 Conduct department-wide audits including:
🟩 Storage room inspections
🟩 Crash cart monthly check
🟩 Expiry monitoring and FEFO verification
🟩 MDV handling compliance
🟩 Temperature logs verification
🟩 Emergency medications stocking
🟩 High-alert medications segregation
🟡 Provide corrective action plans and follow-up reports.

5. Quality Improvement:
Participate in FMEA, RCA, KPI analysis related to medication safety.
🟡 Monitor trends in medication errors and adverse events.
🟡 Propose improvement strategies and track outcomes.

6. Supplier Evaluation:
Participate in supplier performance assessment.
Ensure compliance with procurement standards.
Evaluate packaging, delivery quality, timelines, and documentation accuracy.

7. Reporting & Documentation:
Prepare monthly quality reports.
Document medication-related incidents.
Submit ADR reports to relevant authorities.
Report policy non-compliance issues to management.

Qualifications:
Bachelor’s degree in Pharmacy (PharmD preferred).
Experience in Quality Assurance or Medication Management preferred.
Strong communication and teaching skills.
Audit and report writing skills.
