Frozen shoulder

◼️Also called Adhesive capsulitis, it is an idiopathic condition affecting the shoulder most common in females at their 50's.

◼️It is associated with diabetes, thyroid disease and heart disease, the condition may occur spontaneously or follow minor trauma.

◼️It runs through three phases, painful phase charactarized by severe pain, stiffening phase charactarized by global loss of both active and passive movements, and finally thawing phase charactarized by gradual resolution, the course of disease may take about 2 years.

◼️The pathognomonic sign is complete or almost complete loss of both active and passive external rotation (fig above shows loss of external rotation of left arm).

◼️Plain radiographs are usually normal and used to exclude differential diagnoses.

◼️Treatment include pain relief, including intra-articular steroids, distension of joint capsule with saline, and arthroscopic release of tight capsule, manipulation of the joint under GA is also helpful.
