1. Definition:
Sickle Cell Anemia is a genetic blood disorder characterized by the production of abnormal hemoglobin, called hemoglobin S. This causes red blood cells to take on a rigid, sickle-like shape, impairing their ability to carry oxygen and move through blood vessels.
2. Causes:
โข Genetic Mutation: Caused by a mutation in the HBB gene that encodes for hemoglobin.
โข Inheritance Pattern: It follows an autosomal recessive pattern:
โข A person must inherit two defective genes (one from each parent) to develop the disease.
โข If one defective gene is inherited, the person has sickle cell trait, which generally does not cause symptoms but can be passed on to children.
3. Symptoms:
โข Chronic Anemia: The body struggles to produce enough red blood cells to replace those lost through hemolysis, leading to fatigue, weakness, and pallor.
โข Pain Crises: Severe pain, often referred to as a vaso-occlusive crisis, occurs when sickled cells block blood flow to organs or tissues. It typically affects the chest, back, abdomen, and joints.
โข Swelling: Particularly in the hands and feet, due to blocked blood flow.
โข Frequent Infections: Due to spleen damage, which impairs immune function.
โข Delayed Growth: Reduced oxygen supply affects growth in children and adolescents.
โข Vision Problems: Blockage of small blood vessels in the retina can cause visual impairment.
4. Complications:
โข Stroke: Caused by blockage of blood flow to the brain.
Acute Chest Syndrome: A life-threatening complication characterized by chest pain, fever, and difficulty breathing.
โข Organ Damage: Organs such as the spleen, liver, and kidneys may be damaged over time due to reduced oxygen delivery.
โข Gallstones: Resulting from the breakdown of hemoglobin, leading to excess bilirubin.
โข Leg Ulcers: Chronic sores that can develop on the skin.
โข Pulmonary Hypertension: High blood pressure in the lungs due to restricted blood flow.
5. Diagnosis:
โช๏ธโช๏ธBlood Tests:
โข Hemoglobin Electrophoresis: Identifies abnormal hemoglobin types, including hemoglobin S.
โข Newborn Screening: Many countries routinely screen infants for sickle cell disease at birth.
โข Complete Blood Count (CBC): Can detect anemia and abnormal red blood cell shapes.
โข Genetic Testing: Confirms the presence of mutations in the HBB gene.
6. Treatment:
There is no universal cure, but treatments focus on managing symptoms and preventing complications:
โข Pain Management: Use of painkillers (NSAIDs, opioids) to manage painful crises.
โข Hydroxyurea: A medication that increases fetal hemoglobin (HbF) production, which reduces the number of sickled cells and frequency of pain crises.
โข Blood Transfusions: Used to treat severe anemia, prevent stroke, and reduce complications.
โข Antibiotics and Vaccinations: To prevent infections, especially in children.
โข Bone Marrow/Stem Cell Transplant: The only potential cure, but it's risky and only feasible for a small percentage of patients who can find a suitable donor.
7. Preventive Measures:
โข Hydration: Drinking plenty of water helps prevent sickling of red blood cells.
โข Avoid Extreme Temperatures: Exposure to cold or high heat can trigger crises.
โข Regular Check-ups: Helps in monitoring organ function and preventing complications.
โข Folic Acid Supplements: Supports red blood cell production.

1. Definition:
โข G6PD deficiency is a hereditary condition in which the body does not have enough of an enzyme called glucose-6-phosphate dehydrogenase.
โข It primarily affects the red blood cells, leading to their premature destruction (hemolysis), especially when exposed to certain stressors.
2. Genetics:
โข X-linked recessive disorder.
โข Common in males (since they have only one X chromosome) but females can also be affected if they inherit two defective genes.
โข Prevalence is higher in populations from Africa, the Mediterranean, and Southeast Asia.
3. Function of G6PD:
โข G6PD is crucial in the pentose phosphate pathway, responsible for protecting red blood cells from oxidative damage.
โข It helps in the production of NADPH, which in turn maintains the reduced form of glutathione, a molecule essential for protecting cells against oxidative stress.
4. Symptoms:
โข Many individuals are asymptomatic.
โข Hemolytic Anemia:
โข Sudden destruction of red blood cells triggered by certain factors, leading to symptoms like:
โข Fatigue, jaundice (yellowing of the skin and eyes).
โข Dark urine.
โข Rapid heart rate, shortness of breath.
โข Episodes of hemolysis can be triggered by:
โข Infections: Common cold, pneumonia, hepatitis, etc.
โข Medications: Sulfa drugs, antimalarials, certain antibiotics (like primaquine, dapsone).
โข Foods: Fava beans (favism).
โข Chemical Exposure: Naphthalene (found in mothballs).
5. Diagnosis:
โข G6PD Enzyme Assay: Measures the level of G6PD activity in red blood cells.
โข CBC (Complete Blood Count): May show anemia, elevated reticulocyte count during hemolysis.
โข Peripheral Blood Smear: Shows signs of hemolysis (bite cells, Heinz bodies).
โข Genetic testing (optional) for specific mutations.
6. Treatment & Management:
โข Avoidance: Preventing triggers like certain drugs, foods (fava beans), and stressors (infections).
โข During hemolytic episodes:
โข Blood transfusions (if severe).
โข Supportive care (hydration, monitoring).
โข Newborns: If G6PD deficiency causes jaundice, phototherapy or exchange transfusion may be needed.
โข Genetic counseling for families.
7. Prognosis:
โข Most individuals with G6PD deficiency lead normal lives if triggers are avoided.
โข Severe cases can experience chronic hemolysis, but this is rare.

Anticoagulants are substances that prevent blood clotting (coagulation) by inhibiting the activity of various clotting factors or platelets. They are widely used in laboratories and medical treatment to prevent clot formation.
โช๏ธ Types of Anticoagulants
1. EDTA (Ethylenediaminetetraacetic acid)
โข Mechanism: EDTA chelates calcium ions, which are essential for blood clotting.
โข Use: Commonly used in hematology for complete blood counts (CBC), as it preserves cellular components without causing distortion.
โข Color of tube: Lavender or purple top.
2. Heparin
โข Mechanism: Heparin enhances the activity of antithrombin III, which inactivates thrombin and factor Xa, stopping the clotting process.
โข Use: Suitable for plasma separation in chemistry tests and for blood gas analysis.
โข Color of tube: Green top.
โข Note: It should not be used for hematology tests as it can cause clumping of white blood cells.
3. Sodium Citrate
โข Mechanism: Binds calcium ions, thus preventing clot formation.
โข Use: Mainly used in coagulation studies (PT, APTT) and for blood transfusions.
โข Color of tube: Light blue top.
โข Concentration: 3.2% or 3.8% solution.
4. Oxalates (Potassium Oxalate)
โข Mechanism: Oxalates precipitate calcium ions, preventing the clotting process.
โข Use: Commonly used in glucose testing, as it prevents glycolysis when combined with fluoride.
โข Color of tube: Gray top (usually combined with sodium fluoride).
5. Sodium Fluoride
โข Mechanism: Acts as a weak anticoagulant and a preservative by inhibiting glycolysis.
โข Use: Preferred for glucose testing in conjunction with potassium oxalate.
โข Color of tube: Gray top.
6. ACD (Acid Citrate Dextrose)
โข Mechanism: Citrate binds calcium and prevents clotting, while dextrose preserves red blood cells.
โข Use: Used in blood banking and DNA testing.
โข Color of tube: Yellow top.
โช๏ธ Important Points to Remember
โข Proper mixing: After collecting blood, gently invert the tube 5-10 times to ensure proper mixing of anticoagulant and blood.
โข Impact on results: Anticoagulants can affect certain test results; for instance, EDTA can increase potassium and decrease calcium levels.
โข Shelf life: Use the correct anticoagulant to preserve sample integrity for the required tests.

Introduction
โช๏ธ Glucose Tolerance Test (GTT) is a diagnostic procedure used to assess how well the body processes glucose, which can help in the diagnosis of diabetes mellitus and other disorders related to abnormal glucose metabolism.
Purpose
โซ๏ธ Primary use: Diagnose diabetes mellitus, gestational diabetes, and insulin resistance.
โซ๏ธ Other uses: Diagnose reactive hypoglycemia, acromegaly, and Cushingโs syndrome.
Types of GTT
1. Oral Glucose Tolerance Test (OGTT): The most common form of GTT, where the patient drinks a glucose solution, and blood samples are taken at intervals to measure glucose levels.
2. Intravenous Glucose Tolerance Test (IVGTT): Less common, glucose is administered intravenously, and blood glucose levels are measured at specific intervals.
Procedure
1. Preparation:
- Patient should fast for at least 8 hours before the test.
- Avoid excessive exercise and certain medications before the test, as advised by a doctor.
2. Steps in OGTT:
- Baseline Blood Glucose Level: A blood sample is taken to measure fasting blood glucose.
- Glucose Administration: The patient drinks a solution containing 75g of glucose (or a different dose depending on the patient's condition).
- Blood Samples: Blood samples are taken at various intervals (usually at 30 minutes, 1 hour, 2 hours, and sometimes 3 hours) to measure how the glucose is processed by the body.
3. Steps in IVGTT:
๐ธ Fasting: Similar fasting requirements as OGTT.
๐ธ Glucose Injection: Glucose is injected directly into a vein.
๐ธ Blood Samples: Blood is drawn at regular intervals to measure glucose levels.
Interpretation of Results
โช๏ธ Normal Response:
- Fasting glucose: <100 mg/dL
- 2-hour glucose: <140 mg/dL
โซ๏ธ Impaired Glucose Tolerance (Pre-diabetes):
- Fasting glucose: 100-125 mg/dL
- 2-hour glucose: 140-199 mg/dL
โช๏ธ Diabetes Mellitus:
- Fasting glucose: โฅ126 mg/dL
- 2-hour glucose: โฅ200 mg/dL
Clinical Significance
๐ธ Diabetes Mellitus: A diagnosis is made if the blood glucose levels are consistently higher than normal at the fasting state and after glucose intake.
