History :
9 years old child complaining of Recurrent abd pain
By palpitation epigastric mass
By u/s shows diffuse nodular anterior gastric wall thickening mostly of neoplastic nature

mostly the diagnosis is gastric MALT lymphoma.

Acute lymphoblastic leukemia :

مريض عنده
interstitial lung disease
و
COPD exacerbated by infection
يعني
exacerbated by pneumonia.

الحالة
acute ischemia
painful
no pulse
cold
change in color

الخطوة التالية
Doppler US
and CT angiography
عشان نحدد مكان البتر.

مريض
renal failure
جاء بـ ascites
suspect hepatorenal syndrome ..

العلاج :


- Albumin
- Octreotide
- Metoprolol حسب الضغط.

كيف نعالج الـ ascites :


أولا نمشيه على
spironolacton
لمدة ثلاث ايام
إذا ماخف الـ ascites أو مانفع المريض نعمله
lasix
بكميات كبيرة
ونشوف
إذا مانفع نشوف السبب
وتسمى الحالة هنا
refractory ascites
ونعمل سحب لسوائل البطن.


ايش اسباب ان المريض مايستفيد من العلاج؟
اولا انه مش ملتزم بالعلاج
انه يأكل املاح
انه عنده نقص شديد بالألبومين
او انه دخل ب SBP
أو انه عنده cancer.

مريض مدخن جاء يشتكي من ضيق تنفس وتعرق وحمى وألم بالصدر
عنده COPD
لكن سوينا له ECG
وطلع عنده
ST elevation
يعني MI
سوينا له ..
O2 therapy
Aspirin 75mg 4حبات
plavix 75mg 4حبات
atrova 20mg 4حبات
pantoprazol 40mg infusion
paracetamol 1g infusion
enclax 30mg SC
moxifloxacine infusion

وحولناه PCI.

ECG
لمريض عمره 25 سنة
لا مدخن ولا أي هستوري غير
stressful life

- O2 therapy
- Aspirin 75mg 4حبات
- Plavix 75mg 4حبات
- Atroz 20mg 4حبات
- Pantoprazol 40mg infusion
- Paracetamol 1g infusion
- Clexan 40mg SC
- Fentanyl 50mcg IV slowly
- Ceftriaxon 1g IV stat AST

ويتحول المريض يعمل قسطرة PCI.

طبيعي أي حالة أشوفها قدامي أو معلومة تجي لرأسي اكتبها وانشر
