A 20 years old female patient, known case of pulmonary TB for 2 years she used antiTB for 4 months then stopped because of financial problem, one year ago she developed abdominal distention no Ix or Rx were done or received. 4 months ago she was advised by a doctor to take antiTB drugs again. Today she presented with massive ascites, lower limb edema, dyspnea, fever, cough especially at night, loss of appetite and weight. She has a past drug history of Dytor 20mg for 7 years due to lower limb and face edema (undocumented and unknown dx).
Ix were done in the form of CBC, RBS, s.electrolyte, s.creatinine, abd u/s and CXR.
The Internist advised her to be admitted but she refused because of the poor financial state.
What do you think the differential diagnosis in this case?
