A remarkable era in acute ischemic stroke care is now coming to a close—the period of the “reperfusion revolution.” Over the past few decades, pharmacological and mechanical flow restoration therapies became steadily more effective. By restoring blood flow and terminating the ischemic period, reperfusion salvages those cerebral tissues not yet irreversibly injured. However, the reperfusion strategy in acute stroke care is now reaching its limits. Current endovascular thrombectomy approaches reopen over 85% of occluded arteries, leaving little scope for further advance. And successful reperfusion treatment alone clearly only partially solves the therapeutic challenge. Among patients with ischemic stroke due to large-vessel occlusion who receive endovascular thrombectomy, more than two-thirds nevertheless have a disabled final outcome. Restoring blood flow yields incomplete recoveries because some irreversible injury accrues before recanalization can be achieved and additional injury is generated by reperfusion itself. To fully eradicate stroke-related disability, complementary therapies are needed, including: bridging neuroprotection agents, which allow brain cells to tolerate ischemia until reperfusion is attained; reperfusion neuroprotection agents, which avert reperfusion injury; and neuroreparative agents, which promote postinjury reorganization of neurons and synapses.
