و اخيراً النسخة الجديدة من شيت The most common findings in clinical exam اللي بفضل الله استفادوا منه أكثر من دُفعة و سهل عليهم إمتحان الكلينك Examination stations🩺

الشيت هذا حطيت فيه كل جهدي و خبرتي البسيطة في مجالي الباطنة و رتبته بطريقة تسهل عليكم قرايته و زي ما يبوا دكاترة الكلية
فيه اغلب ال findings اللي مطالبين بيها ك fifith year مع ال discussion اللي حنتسالوا فيها و اللي هيا ال DD و ال Investigations .📝

شيت Examination Chicklist 📝🩺

📝شوية نصائح لامتحان الكلينك🩺

جهزوا البطاقة و البالطو و السماعة و ال tape measure و كل ال instruments اللي
محتاجينها

و انتو داخلين لللجنة ركزوا عالورقة شن مطلوب منكم أديروا ، تلقوها معلقة نهاية ال bed متع البيشنت

طبقوا الخطوات كاملة بداية ب introduce yourself ل عند ما تغطي البيشنت و تشكره
و ديما خليكم مبتسمين و اسألوا المريض لو عنده مكان يوجع فيه او لا من البداية

خليهم صادقين سواء عرفتوا findings او لا ، هما في النهاية 3 لجان ، حتى ما زبطتش في وحدة تزبط في الباقي ، و بعون الله تجيبوهم التلاتة

اعطوا presentation مرتبة

لما تنسالوا في ال DD حطوا في بالكم عمر البيشنت و اعطوا ال most likely DX هيا الأولى
متلا بيشنت عنده signs of UMNL
لقيتها حويجة كبيرة هادي عالاغلب ischemic CVA ، و لو كانت صغيرة في العمر عشرينات تلاتينات هادي عالاغلب MS
و هكذا لكل findings


و ال Investigations تبدوا بتحاليل ال most likely DX و تكملوا على حسب ال DD

◼️في بعض السنوات لما تكون ال findings في لجنة النيورو LMNL ) facial palsy) سألوا حتى عال treatment بالإضافة اللي DD و ال Investigations

📝Bell’s Palsy – Treatment

Prednisone 50–60 mg daily × 5 days → then taper over 5 days (should be started within 72 hrs).

± Antiviral (Acyclovir / Valacyclovir) if severe or Ramsay Hunt suspected.

Eye protection: artificial tears (day), ointment + eye patch (night).

Analgesics (paracetamol/NSAIDs) for pain.

Physiotherapy / facial exercises if prolonged weakness.


👉 Most recover fully in 3–6 months.

1-Vascular/ Stroke , Vasculitis, aneurysm , Cavernous Sinus Thrombosis
2-Infection/ meningitis, encephalitis, brain Abscess
3-Inflammation/ MS

5-Tumeur / space occupying lesion (⬆️ ICP)
6-Systemic diseases/ DM , sarcoidosis

📝Knee joint examination steps

1. Inspection – posture, deformity, swelling, scars, muscle wasting

2. Palpation – warmth, tenderness, effusion (bulge sign, patellar tap), crepitus, quadriceps wasting.

3. Movement – flexion/extension

4. Special tests هادو مش ليكم تبع الجراحة

5.assess the gait

📝DD of Monoarthritis of the Knee joint


🔸️Infectious: Septic arthritis, TB, viral, fungal

🔸️Crystal-induced: Gout, pseudogout (CPPD)

🔸️Traumatic/mechanical: Hemarthrosis, meniscal tear, loose body

🔸️Inflammatory/autoimmune: Early RA, SLE, spondyloarthropathies (reactive, psoriatic, enteropathic)

🔸️Degenerative: Osteoarthritis with effusion

🔸️Neoplastic/infiltrative: PVNS, synovial chondromatosis, metastasis

🔸️Others: Hemophilia, sickle cell, sarcoidosis, amyloidosis

📝What investigations would you like to Elict?

🟨 Arthrocentesis (joint aspiration)
➡️Gross appearance: purulent, bloody, clear, cloudy
➡️Synovial fluid analysis:
◾️WBC count & differential count
> 50,000/mm³ → septic arthritis likely
2,000–50,000/mm³ → inflammatory (RA, gout)
<2,000/mm³ → non-inflammatory (OA)
◾️Gram stain & culture → rule out septic arthritis
◾️Polarized microscopy → urate (needle-shaped, negative birefringence), CPPD (rhomboid, positive birefringence)
Glucose & protein (↓ glucose in infection)

🟨 Blood investigations
◾️CBC, ESR, CRP → inflammation / infection markers
◾️Blood cultures
if septic arthritis suspected
◾️Uric acid level
(may support gout but not diagnostic)
◾️Autoimmune markers
(if chronic/atypical):
Rheumatoid factor (RF), anti-CCP
ANA, HLA-B27 (if spondyloarthropathy suspected)

🟨 Imaging
◾️Plain X-ray (knee)
OA: joint space narrowing, osteophytes
Gout: punched-out erosions (late)
CPPD: chondrocalcinosis
Trauma: fracture, effusion

🟨Ultrasound
Detect effusion, guide aspiration

🟨MRI
If internal derangement (meniscal tear, ligament injury) or neoplastic/synovial pathology suspected


🟨Special tests (when indicated)
◾️Synovial biopsy → TB arthritis, PVNS, tumors
◾️PCR (TB, gonococcal, viral causes) if culture negative but suspicion high
◾️Coagulation profile if hemarthrosis suspected
