Q3_ Why you cannot use irritating substance for testing the Olfactory N ?

تضرب ضربة خفيفة عالجبهة (بالهامر او من غيره) / ال response الطبيعي يصير Eye Blinking مرة وحدة او 2

لو استمر اكثر من هكي معناها / Exaggerated.

🔸️How to test for congugate eye movement?
👇
تحط ايديك الزوز قدام عيون البيشنت على مسافة نص متر
بحيث وحدة تكون مفتوحة 🖐 و التانية تكون مسكرة✊

و تقول للبيشنت مرة يشوف لليد المفتوحة 🖐 ، و مرة يشوف لليد المسكرة ✊

و تراقب حركة العيون هل يتحركوا مع بعض و أمورهم تمام او لا.

How to examine pt with parkinson 👇

◽ تبدا بالFace. 🧑

1_ اديرله test for eye movement 👀
⬅️( اللي عنده parkinson يبدا عندهم impaired eye movement خاصة ال up gaze بسبب ال Bradykynesia )

2_ تقوله يبتسم 😃
⬅️ (طبعاً اللي عنده parkinson ما يقدرش يحرك عضلات وجهه Mask face بسبب ال bradykynesia )

3_ تقوله يقول جملة ( متلاً بسم الله الرحمن الرحيم ) و تقوله مرة يقولها بصوت عالي و مرة يقولها بصوت واتي🗣
⬅️( اللي عنده parkinson ما يقدرش يعلي و يوطي الصوت متعه monotonous speech بسبب ال Bradykynesia )

◽بعدين ال Hand 👐

1_ تشوف لو عنده Tremor او لا
⬅️(اللي عنده parkinson يبدا عنده Resting or static tremor )

2_ تعطيه ورقة و بيرو و تقوله يكتب 📝
⬅️( اللي عندهparkinson الخط متعه يبدا صغير Microgaphia )

3_ ادير test for ms tone
⬅️ ( اللي عنده parkinson حيكون عنده Hypertonia / Rigidity )

◽ بعدين ال lower limb

1_ ادير Test for ms tone
⬅️ ( اللي عنده parkinson حيكون عنده Hypertonia / Rigidity )

2_ توقف المريض و تقوله يمشي assess the gait ، تخليه يمشي مسافة و بعدين تقوله يغير مسار المشي ، يتلفت يمين او يسار و بعدين تقوله يوقف .🚶‍♂️
⬅️ (pt with parkinson have Shuffling gait , difficult to start , difficult to stop , difficult to turns around / Due to Bradykynesia )

شنو الوقت توا ( صبح ، عشية و لا ليل ) ؟
وين انت توا ؟
**فرضاً عرف و قالك اني في المستشفى ، تسأله 👇
علاش جيت للمستشفى ؟
من اللي جابك للمستشفى ؟
**لو فيه معاه Relative تسأله 👇
من هذا للي معاك ؟
**(فرضاً معاه ولده ) تسأله 👇
ولدك قداش عمره ، مواليد كم ؟!
توريه ساعة و لا تليفون و لا بيرو و تسأله هذا شنو ؟
تقوله يعد بالعكس (عشرة / تسعة .....)
تسأله على تاريخ حاجة قديمة (متلاً ، عمي ملحق على الغارة ؟ في اي سنة كانت )
NB:- 1st 3 Questions to assess the orientation for time , place and person .
