💥Aspirin an irreversible COX inhibitor, is particularly effective. Because platelets lack the machinery for synthesis of new protein, inhibition by aspirin persists for several days until new platelets are formed.
Other NSAIDs, which cause a less persistent antiplatelet effect (hours), are not used as antiplatelet drugs and, in fact, can interfere with the antiplatelet effect of aspirin when used in combination with aspirin.

🐰Bivalirudin Used with aspirin in unstable angina when undergoing percutaneous transluminal coronary angioplasty (PTCA).

👉vasopressin acts through V2 receptors to increase the insertion of water channels in the apical membranes of collecting duct cells in the kidney and to thereby provide an antidiuretic effect.
👉Extrarenal V2-like receptors regulate the release of coagulation factor VIII and von Willebrand factor .

☄Desmopressin, a selective agonist of V2 receptors, is administered orally, nasally, or parenterally in patients with pituitary diabetes insipidus and in patients with mild hemophilia A or von Willebrand disease. Because it increases the plasma concentration of von Willebrand factor and factor VIII.

👉Oral anticoagulant that does not require monitoring of PT or INR.

👉Used in atrial fibrillation as an alternative to warfarin

👉Rapidly reversed by idarucizumab (its antidote ).

ال warfarin نستخدمه ضد ال thrombosis ليش لوما نعطوه بعض الاحيان يعمل thrombosis او Hypercoagulability في بداية استخدامه ؟

👉Transient protein C deficiency can be induced when initiating treatment with warfarin because factors VII and protein C have the shortest half lives of the coagulation factors.
👉Hypercoagulability occurs, which may result in dermal vascular thrombosis and skin necrosis.

الصيدلي وضبط جرع الأدوية .. الحلقة (9)

تعدّ تركيبة الـ (Metronidazole + Diloxanide) من التركيبات الدوائية الفعالية في علاج جميع أنواع الأميبا ؛ بسبب وجود دوائين .. أحدهما يقتل الأميبا الموجودة في الأمعاء (وهو الـ Diloxanide) ..والآخر يقتل الأميبا الموجودة في باقي الجسم (وهو الـ Metronidazole) ..

لكني لاحظت وجود أخطاء كثيرة ومتكررة في تحديد جرعة هذا الدواء للبالغين !! حيث يصرّ أحدهم على أنها : (قرص 3 مرات يومياُ) ... والآخر يجعلها (قرص مرتين يومياُ) فقط !!!

الجرعة الصحيحة هي : (( قرصين كل 8 ساعات -بعد الأكل- لمدة 10 أيام)) لأن كل قرص يحتوي فقط على 250 مجم من المترونيدازول + 250 مجم من الديلوكسانيد -كما هو واضح في الصورة-

أي أن القرصين معاً فيهما 500 مجم من الـ (Metronidazole) و 500 مجم من الـ (Diloxanide).. وهذه هي الجرعة الصحيحة ..

إن الاستمرار في إعطاء الدواء بجرع أصغر من اللازم يهدد بفشل المعالجة في المريض .. وربما نشوء سلالات تقاوم الدوائين -على المدى الطويل .. ومن هنا يأتي دور الصيدلي في ضبط جرعة الدواء بالشكل الصحيح .. ليضمن استخدامه بشكل (فعال وآمن) :) ..

ملاحظة:

1) الـ 500 مجم من الـ (Metronidazole) هي أقل جرعة معترف بها في علاج الأميبا (أما أعلى جرعة فهي 800 مجم كل 8 ساعات -بعد الأكل- لمدة 10 أيام أيضاً) ..

2) الجرعة السابقة هي للبالغين الذين لا يعانون من أي قصور كلوي (Renal Impairment) ولا أي اعتلال كبدي (Hepatic Impairment) ..

3) الـ (Metronidazole) يمتلك أيضاً فاعلية ضد الأميبا الموجودة في الأمعاء .. لكن "امتصاصه من الأمعاء إلى الدم" يتم بشكل سريع جداً .. وبالتالي هو -عملياً- لا ينفع لعلاج الـ (Intestinal amebiasis) .. ومن هنا جاءت الحاجة إلى إضافة دواء آخر يتركز في الأمعاء بكميات كافية وهو الـ (Diloxanide) ..
