Best Practice for Silver Nitrate Cautery

1. Preparation

Explain procedure to patient (may cause mild stinging/burning).

Good lighting & suction available.

Topical anesthesia + vasoconstrictor:

4% lidocaine + 0.05% oxymetazoline (or phenylephrine) on cotton for 5–10 min.

Dry the mucosa with gauze/cotton tip before cautery (moisture reduces effectiveness).

2. Identifying Bleeding Point

Usually in Kiesselbach’s plexus (Little’s area) on anterior septum.

Make sure to localize exact vessel/spot (avoid random wide cautery).

3. Application Technique

Use silver nitrate stick – the tip turns brown/black after use.(Eshar)

Touch the bleeding point gently for 5–10 seconds, rolling lightly.

The mucosa should turn grey-white (eschar forms).

Do NOT cauterize both septal sides simultaneously → risk of septal perforation.

Avoid large continuous areas → only pinpoint cautery at bleeding spot.

4. Aftercare

Apply topical antibiotic ointment (e.g. mupirocin, terramycin )5–7 days → keeps area moist, prevents crusting.

Advise no nose blowing, picking, or rubbing for 1 week.

Continue saline irrigation & humidification.

5. Precautions

If active bleeding at time of procedure → first control with vasoconstrictor or light pressure, then cauterize once bleeding stops.

If silver nitrate used excessively → risk of chemical burn and necrosis.

Never use bilateral septal cautery at same level.

#Take home message:
Pinpoint, unilateral, gentle cautery on a dry, localized vessel with silver nitrate stick; follow with lubrication and allergy management.
