ਪੰਜਾਬ ਰਾਜ ਵਿੱਚ ਜਨਮ ਲੈਣ ਵਾਲੇ ਵਿਦਿਆਰਥੀਆਂ ਨੂੰ ਜਨਮ ਅਤੇ ਮੌਤ ਰਜਿਸ਼ਟ੍ਰੇਸ਼ਨ ਦਫ਼ਤਰ ਵੱਲੋਂ ਸੇਵਾ ਕੇਂਦਰ ਰਾਹੀਂ ਜਾਰੀ ਕੀਤਾ ਗਿਆ Date of Birth Certificate ਜੋ ਕਿ Digitised Form ਵਿੱਚ ਹੋਵੇ,ਕੇਵਲ ਉਹ ਹੀ ਜਮ੍ਹਾਂ ਕਰਵਾਉਣਾ ਲਾਜ਼ਮੀ ਹੋਵੇਗਾ। Online Admission Portal (ਆਨਲਾਈਨ ਦਾਖਲਾ ਖਾਰਜ ਰਜਿਸਟਰ) ਵਿੱਚ ਇਸ ਸਰਟੀਫਿਕੇਟ ਦਾ ਰਜਿਸ਼ਟ੍ਰੇਸ਼ਨ ਨੰਬਰ ਭਰਿਆ ਜਾਣਾ ਲਾਜ਼ਮੀ ਹੋਵੇਗਾ ਜੋ ਕਿ ਆਟੋਮੈਟਿਕ ਆਨਲਾਈਨ ਵਿਧੀ ਨਾਲ ਹੀ ਵੈਰੀਫਾਈ ਹੋਵੇਗਾ

*Regarding ACRs 2025-26*
ਸਮੂਹ ਸਕੂਲ ਮੁੱਖੀਆਂ ਦੇ ਧਿਆਨ ਹਿੱਤ ਕੁੱਝ Important Guidelines ਇਸ ਪ੍ਰਕਾਰ ਹਨ, (ਪੂਰਾ message ਪੜ੍ਹਿਆ ਜਾਵੇ ਜੀ):-
1.  *Last Date to initiate/create ACRs 2025-26 was 26.04.2026.* No extension, regarding this, will be further granted by the Department now.
2. All types of grievances (if any), except "Delete my ACR" can be settled before final submission by the Employee to the next Authority.
3. Each and Every employee received SMS massage as *"iHRMS-PB: Employee Name (HRMS ID), Please fill your APAR before 30/04/2026 to avoid last minute rush else it will be auto forwarded to next Authority."* Regarding the above mentioned message, all Non-Teaching Staff (Group-C) must write their Self Appraisal on or before 30.04.2026 and submit to the next Authority through OTP verification. No extension, regarding this, will be granted by the Department.
4. *All Teaching Staff including Principals and HMs don't worry about the above mentioned SMS message.* Further extension, regarding this, will be granted by the Department in due course, taking into consideration the Board Classes 10th & 12th February/March 2026 results. Please don't panic if your ACRs, without writing your Self Appraisal gets auto forwarded to the next authority on 30.04.2026, as these will again be back after further Extension.
5. The most important thing to say for those Schools where the previous DDO retired on 31.03.2026 and mapped as Reporting Officer, All Non-Teaching Staff (Group-C)  and Only Teaching Staff of the Attached Middle Schools must write their Self Appraisal on or before 30.04.2026 and submit to the next Authority through OTP verification. And make sure the previous DDO must fill & Assess Subordinate ACRs through OTP verification before 30.04.2026, *because their HRMS ID will be locked after 30.04.2026.*
